摘要
目的探讨超声参数联合血清人附睾蛋白4(HE4)、人激肽释放酶10(hk10)、DJ-1用于卵巢肿瘤良恶性鉴别的价值。方法选取2020年1月-2023年1月我院收治的100例卵巢肿瘤患者,根据病理结果分为卵巢良性肿瘤组80例和卵巢恶性肿瘤组20例。患者术前均行超声检查及血清HE4、hk10、DJ-1检查,并比较2组各项指标差异,通过ROC分析超声参数、HE4、hk10、DJ-1诊断卵巢恶性肿瘤的效果。结果卵巢良性肿瘤组患者阻力指数(RI)、搏动指数(PI)、S/D以及HE4、hk10、DJ-1水平低于卵巢恶性肿瘤组(P<0.05);ROC曲线结果显示RI、PI、S/D、HE4、hk10、DJ-1诊断卵巢恶性肿瘤的敏感度为0.698、0.721、0.744、0.721、0.814、0.814,联合诊断敏感度为0.953,特异性为0.719、0.789、0.491、0.737、0.684、0.877,联合诊断特异性为0.912;Ⅰ~Ⅱ期卵巢恶性肿瘤患者RI、PI、S/D、HE4、hk10、DJ-1低于Ⅲ~Ⅳ期卵巢恶性肿瘤患者(P<0.05)。结论超声参数、HE4、hk10、DJ-1均可作为鉴别卵巢肿瘤良恶性的指标。
Objective To explore the value of ultrasound parameters combined with serum human epididymal protein 4(HE4),human kallikrein 10(hk10),and DJ-1 in the differentiation of benign and malignant ovarian tumors.Methods 100 patients with ovarian tumors admitted to Fuyang Cancer Hospital from January 2020 to January 2023 were selected and divided into the benign ovarian tumor group(n=80)and the malignant ovarian tumor group(n=20)according to pathological results.All patients underwent preoperative ultrasound examination and the detection of serum HE4,hk10 and DJ-1 levels,and the differences in various indicators between two groups were compared.The diagnostic effect of ultrasound parameters,HE4,hk10,and DJ-1 on ovarian malignant tumors was analyzed through ROC.Results The levels of RI,PI,S/D,HE4,hk10 and DJ-1 in the benign ovarian tumor group were lower than those in the malignant ovarian tumor group(P<0.05);The results of receiver operator characteristic curve(ROC curve)showed that the sensitivity of RI,PI,S/D,HE4,hk10 and DJ-1 in diagnosing malignant ovarian tumor was 0.698,0.721,0.744,0.721,0.814,0.814 respectively,the sensitivity of combined diagnosis was 0.953;The specificity was 0.719,0.789,0.491,0.737,0.684,0.877 respectively,and the specificity of combined diagnosis was 0.912;The RI,PI,S/D,HE4,hk10,and DJ-1 levels in patients with stage I-II malignant ovarian tumor were lower than those in patients with stage III-IV malignant ovarian tumor(P<0.05).Conclusion Ultrasound parameters,HE4,hk10,and DJ-1 can all be used as indicators to differentiate between benign and malignant ovarian tumors.
