芪参活肾汤治疗早中期慢性肾衰竭临床观察

Efficacy observation of Qishen Huoshen decoction on chronic renal failure in the early-and mid-stage

目的:观察芪参活肾汤治疗早中期慢性肾功能衰竭患者的临床疗效。方法:选取符合纳入标准的92例脾肾亏虚、浊毒瘀阻型慢性肾衰竭患者,随机分为两组各46例。对照组予西医常规疗法,治疗组在对照组基础上加服芪参活肾汤中药汤剂,两组均治疗8周。比较两组患者治疗前后血肌酐(Scr)、尿素氮(BUN)、血清胱抑素C(Cys-C)、24小时尿蛋白定量(24hU-TP)水平及中医证候积分、临床疗效情况。结果:治疗组总有效率为84.78%,对照组为60.87%,两组比较,差异有统计学意义(P﹤0.05)。治疗后,治疗组BUN、Scr、24hU-TP、Cys-C水平均低于对照组(P﹤0.05);治疗组患者气短懒言、倦怠乏力、食少纳呆等中医证候评分较治疗前明显降低,且低于对照组(P﹤0.05)。结论:芪参活肾汤治疗脾肾亏虚、浊毒瘀阻型早中期慢性肾衰竭患者临床疗效较好,能有效缓解患者临床症状,改善肾功能。

Objective:To observe the clinical efficacy of Qishen Huoshen decoction on chronic renal failure in the early-and mid-stage.Methods:92 cases of eligible patients with chronic renal failure with syndrome of deficiency of spleen and kidney and obstruction due to turbid toxin and blood stasis were selected and randomly divided into two groups,46 cases in each group.The control group was treated with routine western medicine,while the treatment group was treated plus with Qishen Huoshen decoction of Chinese herbal medicine on the basis of control group.Both groups were treated for 8 weeks.The levels of serum creatinine(Scr),urea nitrogen(BUN),serum cystatin C(Cys-C)and 24-hour urinary total protein(24hU-TP)as well as TCM syndrome score and clinical efficacy of patients were compared between two groups before and after treatment.Results:The total effective rate was 84.78%in the treatment group and 60.87%in the control group,and the comparative difference between two groups was statistically significant(P<0.05).After treatment,the levels of BUN,Scr,24hU-TP and Cys-C of treatment group were lower than those of control group(P<0.05);TCM syndrome scores of patients in treatment group,such as shortness of breath and lazy talk,burnout and fatigue,less eating and anorexia,etc.were significantly reduced compared with before treatment,and which was lower than in control group(P<0.05).Conclusion:The clinical efficacy of Qishen Huoshen decoction on chronic renal failure in the early-and mid-stage and with syndrome of deficiency of spleen and kidney and obstruction due to turbid toxin and blood stasis is better,which can effectively improve the clinical symptoms and renal function of patients.
