循环纤维细胞在坠积性肺炎中的检测及意义

摘要 目的:定量分析循环纤维细胞(CFs)在坠积性肺炎(HP)患者外周血中的含量,探究其与疾病严重程度的相关性及在发病风险预测中的应用价值。方法:选取2023年8月-2023年11月潍坊市人民医院重症医学科新发HP患者49例,另选取同期查体健康对照(CON)人员35名;采用流式细胞术检测外周血CD45+细胞中CFs(CD45+ColⅠ+细胞)比例,分析其与白细胞计数(WBC)、中性粒细胞百分比(NE%)、中性粒细胞绝对值(NE)、C反应蛋白(CRP)等感染指标相关性;构建ROC曲线评估CFs含量在HP辅助诊断中的价值。结果:HP患者中CFs比例(15.81±10.20)%显著高于健康人群(7.54±3.31)%(P<0.0001)。ROC曲线显示,CFs比例对HP具有辅助诊断价值(AUC为:0.746,cutoff值为:11.32%)。Spearman相关性分析显示,CFs比例与WBC(r=-0.3413,P=0.0164)、NE%(r=-0.2881,P=0.0495)、NE(r=-0.3352,P=0.0185)明显负相关。结论:HP患者体内CFs比例上调,具有较好的疾病鉴别价值,与临床感染指标负相关,提示CFs可能在HP临床诊疗中具有重要应用价值。 Objective:To quantitatively analyze circulating fibroblasts(CFs)in the peripheral blood of patients with hemostatic pneumonia(HP),with the objective of investigating the potential correlation between CF levels and disease severity,as well as assessing the utility of CFs in predicting morbidity risk.Methods:A cohort of 49 HP patients were recruited from the Department of Intensive Care Medicine at Weifang People's Hospital between August 2023 and November 2023,alongside 35 healthy control(CON)individuals during the same timeframe.The proportion of CFs(CD45+ColI+cells)within peripheral blood CD45+cells was assessed using flow cytometry,and its association with white blood cell(WBC),neutrophil percentage(NE%),neutrophil(NE),and C-reactive protein(CRP).ROC was constructed to evaluate the value of CFs in the assisted diagnosis of HP.Results:The percentage of CFs in HP patients(15.81±10.20)%was significantly higher than that in the healthy population(7.54±3.31)%(P<0.0001).The ROC curve showed that CFs served as a useful diagnostic marker for HP(AUC:0.746,cutoff value:11.32%).Furthermore,spearman correlation analysis revealed significant negative correlations between the percentage of CFs and white blood cell(r=-0.3413,P=0.0164),neutrophil percentage(r=-0.2881,P=0.0495),and neutrophil(r=-0.3352,P=0.0185).Conclusion:In HP patients,the CFs is up-regulated and inversely associated with clinical infection indicators.Besides,CFs may serve as a potential marker for differential disease diagnosis These findings suggest that CFs could hold significant clinical utility in the diagnosis and management of HP.
