孕妇血清内脂素水平与糖代谢异常严重程度的相关性分析

Analysis on the correlation between serum visfatin level and the severity of abnormal glucose metabolism in pregnant women

摘要 目的 通过检测孕妇口服葡萄糖耐量试验(OGTT)血糖、内脂素及胰岛素水平,了解血清内脂素水平与孕妇糖代谢异常严重程度的关系。方法 选取2020年6月—2021年5月在杭州市妇产科医院门诊常规产检并有分娩意向的孕24~28周的孕妇共66例,检测空腹和服糖后1 h、2 h血糖、内脂素及胰岛素水平。按照孕妇血糖结果分为4组:血糖均正常的为对照组(22例),血糖升高1次的为GDM1组(14例),血糖升高2次的为GDM2组(22例),血糖升高3次的为GDM3组(8例)。比较组间孕妇的临床资料、血糖、内脂素及胰岛素水平,采用Pearson相关性检验分析内脂素与血糖、胰岛素水平及胰岛素拮抗指数(HOMA-IR)的相关性。结果 4组孕妇的HOMA-IR、空腹血糖(G0)、服糖后1 h血糖(G1)、服糖后2 h血糖(G2)、空腹内脂素(V0)、服糖后1 h内脂素(V1)、服糖后2 h内脂素(V2)、空腹胰岛素(I0)、服糖后1 h胰岛素(I1)及服糖后2 h胰岛素(I2)比较差异均有统计学意义(均P<0.05)。HOMA-IR与V0(r=0.699,P<0.05)、V1(r=0.690,P<0.05)、V2(r=0.555,P<0.05)均呈正相关。V0与G0及I0均呈正相关(r=0.738,0.627,均P<0.05),V1与G1呈正相关(r=0.551,P<0.05),与I1无相关性(r=-0.216,P>0.05);V2与G2呈正相关,与I2呈负相关(r=0.275,-0.252,均P<0.05)。结论 GDM患者糖代谢异常严重程度可能与空腹内脂素水平相关,临床可通过测定空腹内脂素水平来评估GDM患者糖代谢异常严重程度。 ObjectiveTo explore the relationship between serum visfatin level and the severity of abnormal glucose metabolism in preg-nant women by detecting the levels of serum glucose,visfatin,and insulin of pregnant women who underwent oral glucose tolerance test(OCTT).Methods A total of 66 pregnant women at 24-28 gestational weeks were selected from June 2020 to May 2021 for routine ante-natal check-up in Outpatient Department of Hangzhou Women's Hospital.Serum glucose,visfatin,and insulin levels were detected on fast-ing,1 hour and 2 hours after taking sugar.According to blood glucose results,the pregnant women were divided into 4 groups:normal control group(22 cases),GDM1 group(14 cases),GDM2 group(22 cases),and GDM3 group(8 cases).The clinical data,blood glucose,vis-fatin and insulin levels of pregnant women in different groups were compared,and the correlations between visfatin and blood glucose,insu-lin,and HOMA-IR levels were analyzed by Pearson correlation test.Results There were statistically significant differences in the levels of HOMA-IR,fasting blood glucose(GO),one-hour blood glucose after taking sugar(C1),two-hour blood glucose after taking sugar(G2),fasting visfatin(VO),one-hour visfatin after taking sugar(V1),two-hour visfatin after taking sugar(V2),fasting insulin(10),one-hour insulin after taking sugar(I1),and two-hour insulin after taking sugar(12)among the 4 groups(all P<0.05).HOMA-IR was positively correlated with V0(r=0.699,P<0.05),V1(r=0.690,P<0.05),V2(r=0.555,P<0.05).V0 was positively correlated with C0 and I0(r=0.738,0.627,all P<0.05).V1 was positively correlated with G1(r=0.551,P<0.05),but not with Il(r=-0.216,P>0.05).V2 was positively correlated with G2 and negatively correlated with 12(r=0.275,-0.252,all P<0.05).Conclusion The severity of ab-normal glucose metabolism in GDM patients may be related to fasting visfatin level.The severity of abnormal glucose metabolism in GDM pa-tients can be evaluated clinically by measuring fasting visfatin level.
