肥胖影响慢性疼痛及分子机制研究进展

Research progress on the molecular mechanism of obesity in chronic pain

摘要 肥胖与慢性疼痛是突出的全球健康问题。临床研究表明,慢性疼痛的发病率在肥胖人群中显著升高;动物实验表明,肥胖增加疼痛敏感性,但具体作用机制尚未见阐明。文章从炎症、免疫、神经内分泌、肠道微生物、信号分子及通路等方面对近年来肥胖影响慢性疼痛的作用机制研究进行阐述,旨在优化临床肥胖患者疼痛防治策略,并为进一步研究提供思路。 Obesity and chronic pain are prominent global health problems.Clinical studies have shown that the incidence of chronic pain significantly increases in obese people.Animal evidence suggests that obesity increases pain sensitivity,but the detailed mechanisms have not been completely elucidated.This article reviews the recent mechanisms of obesity affecting chronic pain from the aspects of inflammation,immunity,neuroendocrine,intestinal microbiome and pathophysiological signaling pathways,in order to opti⁃mize the prevention and treatment of pain in obese patients and provide thoughts for further research.
