高频超声在聚丙烯酰胺水凝胶隆乳剂取出前后的应用价值

Application value of high frequency ultrasound before and after removal of polyacrylamide hydrogel in breasts

目的探讨聚丙烯酰胺水凝胶隆乳剂(PAHG)取出前后行高频超声检查的临床价值。方法回顾性分析2015年5月至2021年7月于河南省人民医院就诊的拟取出PAHG的患者97例共194个假体手术前后的超声资料。所有患者PAHG取出术前均行高频超声检查观察假体解剖层次,分析假体回声,观察是否存在假体破裂、渗漏、移位、包膜挛缩、散在硬结、感染等隆乳术并发症;术后随访,行高频超声检查监测聚丙烯酰胺水凝胶取出后乳腺的超声表现。结果PAHG取出术前超声结果显示,97例194个假体中,假体双侧不对称者124个,触及散在硬结者47个,假体移位40个,渗漏22个,包膜挛缩95个,感染8个。聚丙烯酰胺水凝胶取出后超声扫查显示,4个取出彻底,余190个假体存在不同程度的残留。结论高频超声能够精准诊断PAHG取出术前的隆乳术并发症,及时准确评估PAHG取出术的手术效果及残留物的有无及多少。

Objective To investigate the clinical value of high frequency ultrasonography before and after removal of polyacrylamide hydrogel(PAHG)in breasts.Methods The ultrasound data of 97 patients with a total of 194 prostheses who were prepared to have PAHG removed in Henan Provincial People’s Hospital from May 2015 to July 2021 were retrospectively analyzed.Before PAHG removal,all patients underwent high frequency ultrasound examination to observe the anatomical level of the prosthesis,the prosthesis echo was analyzed to observe the occurrence of complications of breast augmentation surgery,including prosthesis rupture,leakage,displacement,capsular contracture,scattered induration,infection.After the postoperative follow-up,high-frequency ultrasonography was performed to monitor the ultrasonic manifestations of the breast after the removal of PAHG.Results The ultrasound results before PAHG removal revealed that 124 of 194 prostheses in 97 patients had bilateral asymmetry,47 cases were diagnosed with scattered induration by touching,40 cases had displacement,22 cases had leakage,95 cases had capsular contracture,and 8 cases had infection.Ultrasonic scan after removal of PAHG revealed that 4 prostheses were removed completely,and the remaining 190 prostheses had different degrees of residue.Conclusions High-frequency ultrasound can accurately diagnose the complications of breast augmentation before PAHG removal,moreover,it can timely and accurately predict the surgical effect,and evaluate the presence and amount of residue after PAHG removal.
