瓜蒌牛蒡汤内服联合红外线照射和通乳手法治疗急性乳腺炎的效果观察

Clinical effect of oral administration of Gualou Niubang decoction combined with infrared radiation and lactation manipulation in the treatment of acute mastitis

目的分析瓜蒌牛蒡汤内服联合红外线照射、通乳手法治疗急性乳腺炎的临床效果。方法队列研究。抽取2021年3月至2023年5月平顶山市妇幼保健院收治的96例急性乳腺炎患者作为研究样本,其中48例接受红外线照射+通乳手法治疗的患者设为对照组,另外48例在接受红外线照射+通乳手法治疗基础上口服瓜蒌牛蒡汤治疗的患者设为观察组。比较两组肿块消失时间、乳房肿痛消失时间、治疗前后疼痛乳房疼痛评分及中医症候积分变化情况。结果观察组肿块消失时间、乳房肿痛消失时间均短于对照组(P均<0.05)。治疗前,两组乳房疼痛评分比较差异未见统计学意义(P>0.05),治疗后第2、4、6、8天,观察组乳房疼痛评分均低于对照组(P均<0.05)。治疗前,两组急性乳腺炎中医症候积分比较差异未见统计学意义(P>0.05),治疗后观察组乳房胀痛、乳汗排泄、乳腺结节积分均低于对照组(P均<0.05)。结论瓜蒌牛蒡汤内服+红外线照射+通乳手法治疗急性乳腺炎的疗效理想,可显著缩短乳腺肿块消失时间,降低乳房疼痛程度。

Objective To analyze the effect of oral administration Gualou Niubang decoction combined with infrared radiation and lactation promotion manipulation therapy on acute mastitis.Methods This study was a cohort study.A total of 96 patients with acute mastitis treated in Pingdingshan Maternal and Child Health Care Hospital from March 2021 to may 2023 were selected as the research samples;among them,48 patients who received infrared irradiation combined with lactation manipulation were set as the control group,and the other 48 patients who received infrared irradiation combined with lactation manipulation therapy and oral Gualou Niubang decoction were set as the observation group.The mass disappearance time,breast swelling and pain disappearance time,pain breast pain score and traditional Chinese medicine(TCM)symptom score before and after treatment were compared between the two groups.Results The disappearance time of mass and breast swelling and pain in the observation group were shorter than those in the control group(all P<0.05).There was no significant difference in breast pain scores between the two groups before treatment(P>0.05).The breast pain scores of the observation group were lower than those of the control group on the 2nd,4th,6th and 8th days after treatment(all P<0.05).Before treatment,there was no statistically significant difference in the TCM symptom score of acute mastitis between the two groups(P>0.05);after treatment,the breast pain,breast sweat excretion,and breast nodule score of the observation group were lower than those of the control group(all P<0.05).Conclusions The curative effect of oral anministration of Gualou Niubang decoction orally combined with infrared radiation and lactation manipulation in the treatment of acute mastitis is ideal,which can significantly shorten the disappearance time of breast lumps and reduce the degree of breast pain.
