Analysis of Costs and Influencing Factors of 3520 Inpatients with Lumbar Disc Herniation

目的探讨中医医院腰椎间盘突出住院患者费用构成及其影响因素,为腰椎间盘突出症患者出院费用的控制提供参考。方法选取某中医医院病案系统2022年1月1日-2022年12月31日出院的3520例腰椎间盘突出症住院患者的总费用及构成情况,采用单因素方差分析和多元逐步回归分析法对住院费用的影响因素进行统计分析。结果3520例腰椎间盘出院患者人均住院费用为10974.45元,材料费用占比最高,达到31.24%;药品费和检查费次之,分别占比16.85%和16.76%。多因素分析结果显示,是否手术、年龄、性别、住院天数是影响腰椎间盘突出症住院患者住院费用的主要影响因素,其中,是否手术对出院费用影响最大,比较手术和非手术治疗的出院患者,手术患者平均住院费用增加3458.25元,差异具有统计学意义(P<0.05)。结论腰椎间盘突出症出院患者住院费用较高,产生的经济负担较重,医院要全面提高医疗技术水平,不断研究新技术,做到早发现早治疗,科学控制医疗费用的支出。

Objectives This study aims to explore the cost structure and influencing factors of inpatients with lumbar disc herniation in traditional Chinese medicine hospitals,in order to provide a reference for effective control of discharge costs for patients with lumbar disc herniation.Methods The total cost and composition of 3520 inpatients with lumbar disc herniation who were discharged from the medical record system of a city's traditional Chinese medicine hospital from January 1 to December 31,2022 were selected.One-way analysis of variance and multiple stepwise regression analysis were used to conduct statistical analysis on the influencing factors of hospitalization costs.Results The per capita hospitalization cost of 3,520 lumbar disc discharge patients was 10,974.45 yuan,with material costs accounting for the highest proportion,reaching 31.24%.Drug fees and inspection fees followed,accounting for 16.85%and 16.76%respectively.Multi-factor analysis results showed that surgery,age,gender,and length of stay were the main factors affecting the hospitalization costs of inpatients with lumbar disc herniation.Among them,whether surgery was performed had the greatest impact on discharge costs.Comparing the discharged patients with surgical and non-surgical treatment,the average hospitalization cost of surgical patients increased by 3,458.25 yuan,and the difference was statistically significant(P<0.05).Conclusions Patients discharged from the hospital with lumbar disc herniation have higher hospitalization expenses and heavier financial burdens.Hospitals must comprehensively improve medical technology,continuously research new technologies,achieve early detection and early treatment,and scientifically control medical expenses.
