摘要
目的:探讨乙酰半胱氨酸联合布地奈德雾化吸入治疗小儿支原体肺炎的效果。方法:选择2019年4月—2022年7月北京京都儿童医院急诊科收治的120例支原体肺炎患儿作为研究对象。按照随机数表法将其分为常规组、联合组,各60例。常规组给予乙酰半胱氨酸治疗;联合组给予乙酰半胱氨酸联合布地奈德雾化吸入治疗。比较两组的血清炎症因子水平、临床疗效、临床症状及不良反应发生情况。结果:联合组体温正常时间、咳嗽消失时间、气促消失时间、肺部湿啰音消失时间均早于常规组,差异有统计学意义(P<0.05)。治疗前,两组血清炎症因子水平、血气指标比较,差异无统计学意义(P>0.05);治疗后,两组白细胞介素-6(IL-6),血清白细胞介素-13(IL-13)及肿瘤坏死因子-α(TNF-α)水平低于治疗前,且联合组均低于常规组,差异有统计学意义(P<0.05);治疗后,两组血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))高于治疗前,动脉血二氧化碳分压(PaCO_(2))低于治疗前,且联合组SaO_(2)、PaO_(2)高于常规组,PaCO_(2)低于常规组,差异有统计学意义(P<0.05)。联合组治疗有效率为95.00%,高于常规组的70.00%,差异有统计学意义(P<0.05)。联合组不良反应发生率低于常规组,差异有统计学意义(P<0.05)。结论:布地奈德联合乙酰半胱氨酸治疗小儿支原体肺炎的效果更好,可有效调节炎症因子的产生及释放,改善肺功能,具有较高的安全性。
Objective:To investigate the effect of Acetyl Cysteine combined with Budesonide aerosol inhalation in the treatment of Mycoplasma pneumonia in children.Method:A total of 120 children with Mycoplasma pneumonia admitted to the Emergency Department of Beijing Jingdu Children's Hospital from April 2019 to July 2022 were selected as the study objects.According to the method of random number table,they were divided into conventional group and combined group,with 60 cases in each group.The conventional group was treated with Acetyl Cysteine;and the combined group treated with Acetyl Cysteine combined with Budesonide aerosol inhalation therapy.The levels of serum inflammatory factors,clinical efficacy,clinical symptoms and occurrence of adverse reactions were compared between the two groups.Result:The normal temperature time,cough disappearance time,shortness of breath disappearance time and lung wet rales disappearance time of combined group were earlier than those of conventional group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in serum inflammatory factors and blood gas indexes between the two groups(P>0.05).After treatment,the levels of interleukin-6(IL-6),serum interleukin-13(IL-13)and tumor necrosis factor-α(TNF-α)in two groups were lower than those before treatment,and which in the combined group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After treatment,the blood oxygen saturation(SaO_(2))and arterial partial pressure of oxygen(PaO_(2))of the two groups were higher than those before treatment,and the arterial partial pressure of carbon dioxide(PaCO_(2))of the two groups were lower than those before treatment,and the SaO_(2) and PaO_(2) of the combined group were higher than those of the conventional group,and the PaCO_(2) was lower than that of the conventional group,the differences were statistically significant(P<0.05).The effective rate of combination group was 95.00%,which was higher than 70.00%of conventional group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in combination group was lower than that in conventional group,and the difference was statistically significant(P<0.05).Conclusion:Budesonide combined with Acetyl Cysteine can effectively regulate the production and release of inflammatory factors,improve lung function,and have a high safety in the treatment of Mycoplasma pneumonia in children.
