凝血功能检测在脊柱退行性病变患者术后DVT中的应用价值

Application Value of Coagulation Function in DVT of Patients with Spinal Degenerative Diseases after Surgery

目的:分析凝血功能检测在脊柱退行性病变患者术后深静脉血栓(DVT)中的应用价值。方法:回顾性分析2021年1月—2023年1月漳州市中医院收治的80例脊柱退行性病变患者的临床资料。根据术后1周内彩色多普勒超声检查结果将其分为非DVT组(n=53)和DVT组(n=27)。检测所有患者D-二聚体(D-D)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、国际标准化比值(INR)。比较两组D-D、PT、APTT、TT、FIB、INR,分析D-D、PT、APTT、TT、FIB、INR在脊柱退行性病变术后DVT中的预测价值。结果:DVT组D-D、FIB均高于非DVT组,PT、APTT、TT均短于非DVT组,INR低于对照组,差异有统计学意义(P<0.05)。受试者工作特征(ROC)曲线结果显示,D-D、PT、APTT、TT、FIB、INR联合检测预测脊柱退行性病变术后DVT的AUC高于6项单独预测。结论:D-D、PT、APTT、TT、FIB、INR联合检测能够有效预测脊柱退行性病变患者术后DVT。

Objective:To analyze the application value of coagulation function in deep vein thrombosis(DVT)of patients with spinal degenerative diseases after surgery.Method:The clinical data of 80 patients with spinal degeneration admitted to Zhangzhou Traditional Chinese Medicine Hospital from January 2021 to January 2023 were retrospectively analyzed.According to the results of color Doppler ultrasonography within 1 week after surgery,the patients were divided into non DVT group(n=53)and DVT group(n=27).D-D,prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB),and international standardized ratio(INR)were measured in all patients.The D-D,PT,APTT,TT,FIB,and INR in the two groups were compared.The predictive value of D-D,PT,APTT,TT,FIB,and INR in postoperative DVT of spinal degenerative diseases was analyzed.Result:The D-D and FIB of the DVT group were higher than those of the non DVT group,while PT,APTT,and TT were shorter than those of the non DVT group.The INR was lower than that of the control group,and the difference was statistically significant(P<0.05).The receiver operating characteristic(ROC)curve results showed that the combined detection of D-D,PT,APTT,TT,FIB,and INR predicted a higher AUC for postoperative DVT in spinal degenerative lesions compared to the six individual predictions.Conclusion:The combined detection of D-D,PT,APTT,TT,FIB and INR can effectively predict DVT in patients with spinal degenerative diseases after surgery.
