摘要
目的探讨系统性红斑狼疮合并细菌性感染患者信号转导激活因子(STAT)1、STAT3、白细胞介素(IL)-1βmRNA相对表达量及其与辅助性T细胞亚群1/2(Th1/Th2)平衡的关系。方法选取2020年1月-2023年6月北大医疗海洋石油医院收治的30例系统性红斑狼疮合并细菌性感染患者和35例系统性红斑狼疮未合并细菌性感染患者,分别为感染组和非感染组,比较两组患者外周血Th1、Th2及STAT1、STAT3、IL-1β表达水平,分析STAT1、STAT3、IL-1β与Th1、Th2的相关性,受试者工作特征(ROC)曲线分析STAT1、STAT3、IL-1β对系统性红斑狼疮合并细菌性感染的诊断价值。结果感染组Th1、Th1/Th2表达水平低于非感染组(P<0.05),而Th2、STAT1、STAT3、IL-1β表达水平高于非感染组(P<0.05);STAT1、STAT3、IL-1β与Th1、Th1/Th2呈负相关(P<0.05),与Th2呈正相关(P<0.05);STAT1、STAT3、IL-1β诊断系统性红斑狼疮合并细菌性感染的曲线下面积(AUC)为0.950,敏感度为86.67%,特异度为94.29%。结论系统性红斑狼疮患者合并细菌性感染后,外周血Th1、Th1/Th2表达水平降低,Th2、STAT1、STAT3、IL-1β表达水平升高;STAT1、STAT3、IL-1β与Th1、Th1/Th2呈负相关,与Th2呈正相关;STAT1、STAT3、IL-1β联合对系统性红斑狼疮合并细菌性感染的诊断价值较高。
OBJECTIVE To explore the relative expression levels of signal transduction activator(STAT)1,STAT3 and interleukin(IL)-1βmessenger RNA(mRNA)in the systemic lupus erythematosus(SLE)patients complicated with bacterial infection and analyze the association with balance of helper T cell 1(Th1)/helper T cell 2(Th2).METHODS A total of 30 SLE patients with bacterial infection and 35 SLE patients without bacterial infection who were treated in Peking University Medical Offshore Oil Hospital from Jan 2020 to Jun 2023 were enrolled in the study and were respectively assigned as the infection group and the non-infection group.The expression levels of peripheral blood Th1,Th2,STAT1,STAT3 and IL-1βwere compared between the two groups of patients,the association between STAT1,STAT3,IL-1βand Th1,Th2 was observed,and the values of STAT1,STAT3 and IL-1βin diagnosis of bacterial infection in the SLE patients were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The expression levels of Th1 and Th1/Th2 of the infection group were significantly lower than those of the non-infection group(P<0.05),while the expression levels of Th2,STAT1,STAT3 and IL-1βof the infection group were significantly higher than those of the non-infection group(P<0.05).The STAT1,STAT3 and IL-1βwere negatively correlated with the Th1 and Th1/Th2(P<0.05)but were positively correlated with Th2(P<0.05).The area under curve(AUC)of STAT1,STAT3 and IL-1βwas 0.950 in diagnosis of bacterial infection in the SLE patients,with the sensitivity 86.67%,the specificity 94.29%.CONCLUSION The SLE patients complicated with bacterial infection show the decline of expression levels of peripheral blood Th1 and Th1/Th2 and the rise of expression levels of Th2,STAT1,STAT3 and IL-1β.STAT1,STAT3 and IL-1βare negatively correlated with Th1 and Th1/Th2 but were positively correlated with Th2.The joint detection of STAT1,STAT2 and IL-1βhas high value in diagnosis of the bacterial infection in the SLE patients.
