基于“脾-肠”相关防治糖脂代谢性疾病

Discussion on Treatment of Glycolipid Metabolic Diseases Based on Theory of Spleen and Intestine Being Correlated

糖、脂是精微物质的主要成分,其代谢失常,容易导致各种病变。中医脾运化水谷精微与西医糖脂能量代谢存在高度一致性,故现代中医学认为糖脂代谢性疾病其病位在脾,主要责之脾失健运,而脾失健运与肠道的失司密切相关,脾、肠不调导致的病理产物和功能障碍是糖脂代谢紊乱的关键。从中医“脾-肠”相关出发,更有助于糖脂代谢性疾病的治疗,治疗上应以恢复脾之健运为主,脾气健运,湿浊痰瘀可除。肠道菌群可将食物分解、转化为营养物质,在糖脂代谢中发挥着巨大的作用,这与“脾”运化水谷精微的功能类似,健脾中药在改善脾肠功能异常的同时,也会调节肠道菌群,故肠道菌群失衡是脾失健运的微观体现,亦是现代研究对中医“脾-肠”相关的丰富和补充。因此结合中医文献理论和现代科学研究,深入探讨中医“脾-肠”相关、肠道菌群与糖脂代谢之间的关系,以及从“脾-肠”相关出发治疗糖脂代谢紊乱的相关研究,以期为中西医结合防治糖脂代谢性疾病提供新的思路。

Sugar and lipid are the main components of nutrients,and their metabolic malfunction can easily lead to a variety of pathologies.The transportation and transformation of grain and water of Traditional Chinese medicine spleen are highly consist-ent with the sugar and lipid energy metabolism in western medicine.According to modern Chinese medicine,glycolipid metabolic diseases are located in spleen,and the main cause of the disease is spleen failing to transport and transform,which is closely relat-ed to intestinal disorders.Pathological products and dysfunctions resulting from spleen and intestinal disorders are the key to dis-orders of glucolipid metabolism.From the traditional Chinese medicine theory of spleen and intestine being correlated,it is more helpful to the treatment of glycolipid metabolic diseases.The treatment should be to strengthen spleen.When the spleen restores governing transportation and transformation,turbid,dampness,phlegm and blood stasis can be removed.Intestinal flora can de-compose and convert food into nutrients,and plays a great role in glycolipid metabolism,which is similar to the transporting and transforming function of spleen.Traditional Chinese medicine for invigorating spleen not only improves the abnormal function of spleen and intestines,but also regulates the intestinal flora.Therefore,the imbalance of intestinal flora is a micro manifestation of spleen dysfunction,and it is also a rich and supplementary supplement to modern research on the“spleen-intestine”being relat-ed in traditional Chinese medicine.This paper combines the theory of traditional Chinese medicine literature and modern scientif-ic research to discuss the relationship of“spleen-intestine”,intestinal flora and glucose and lipid metabolism,as well as the clinical treatment and related research of glucolipid metabolism disorders from“spleen-intestine”being correlated,in order to provide new ideas for the prevention and treatment of glucolipid metabolism diseases by combining traditional Chinese and West-ern medicine.
