摘要
目的探讨甲泼尼龙联合红霉素治疗儿童肺炎支原体大叶性肺炎的临床效果。方法选取涟水县人民医院于2021年11月—2023年11月收治的100例支原体大叶性肺炎患儿为研究对象,以随机数表法分为两组,对照组(n=50)予红霉素治疗,观察组(n=50)联合甲泼尼龙治疗,比较两组临床疗效、症状改善时间、治疗前后肺功能及炎症反应。结果观察组治疗总有效率高于对照组(96.00%vs 82.00%),差异有统计学意义(χ^(2)=5.005,P<0.05)。观察组症状改善时间[发热(2.52±0.74)d、咳嗽(6.50±1.22)d、肺啰音(5.95±1.26)d]均短于对照组,差异有统计学意义(t=7.126、4.836、5.050,P均<0.05)。治疗后,观察组用力肺活量、第1 s用力呼气量、呼气峰流速优于对照组,C反应蛋白、白细胞介素-6水平低于对照组,差异有统计学意义(P均<0.05)。结论儿童肺炎支原体大叶性肺炎采用红霉素联用甲泼尼龙治疗可提高疗效,加快康复,改善肺功能并减轻炎症反应。
Objective To investigate the clinical effect of methylprednisolone combined with erythromycin in the treatment of mycoplasma pneumoniae lobar pneumonia in children.Methods A total of 100 children with mycoplasma lobar pneumonia admitted to Lianshui County People's Hospital from November 2021 to November 2023 were selected as the study objects and divided into two groups by random number table method:control group(n=50)was treated with erythromycin;observation group(n=50)was treated with methylprednisolone.The clinical efficacy,symptom improvement time,lung function and inflammatory response before and after treatment were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(96.00%vs 82.00%),and the difference was statistically significant(χ^(2)=5.005,P<0.05).The improvement time of symptoms in the observation group[fever(2.52±0.74)d,cough(6.50±1.22)d and pulmonary rales(5.95±1.26)d]were shorter than those in the control group,and the differences were statistically significant(t=7.126,4.836,5.050,all P<0.05).After treatment,the forced vital capacity,forced expiratory volume in the first second and peak expiratory flow rate in the observation group were better than those in the control group,and the levels of C-reactive protein and interleukin-6 were lower than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Erythromycin combined with methylprednisolone in the treatment of mycoplasma pneumoniae lobar pneumonia in children can improve the curative effect,accelerate recovery,improve lung function and reduce inflammation.
