多西环素在儿童大环内酯类耐药支原体肺炎治疗中的效果分析

摘要 目的探讨应用多西环素治疗儿童大环内酯类耐药肺炎支原体肺炎(Mycoplasma Pneumoniae Pneumo-nia,MPP)的效果。方法选取宁波大学附属第一医院于2022年10月—2023年10月收治的80例大环内酯类耐药MPP患儿为研究对象,根据随机数表法分成观察组(n=40)、对照组(n=40)。两组均给予止咳化痰、补水补液等基础干预,在此基础上对照组应用阿奇霉素治疗7 d,观察组应用多西环素治疗7 d。观察并分析两组的临床治疗效果。结果观察组临床总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(χ^(2)=5.164,P<0.05)。观察组症状缓解时间明显短于对照组,差异有统计学意义(P<0.05)。治疗7 d后,血清炎症因子指标水平如血清C反应蛋白、白细胞介素-6、白细胞介素-17水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P均<0.05)。白细胞介素-10水平高于治疗前,且观察组高于对照组,差异有统计学意义(P均<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论对于儿童大环内酯类耐药MPP,给予多西环素治疗效果明显,可促进患儿症状缓解,降低炎症因子指标水平,安全性较好。 Objective To investigate the effect of doxycycline in the treatment of macrolide resistant mycoplasma pneumoniae pneumonia(MPP)in children.Methods A total of 80 children with macrolide resistant MPP treated in the First Affiliated Hospital of Ningbo University from October 2022 to October 2023 were selected as the study objects,and were divided into observation group(n=40)and control group(n=40)according to random number table method two groups.Both groups were given basic intervention such as relieving cough and resolving phlegm,replenishing water and liquid.On this basis,the control group was treated with azithromycin for 7 days,and the observation group was treated with doxycycline for 7 days.The clinical therapeutic effect was observed and analyzed between the two groups.Results The clinical total effectiveness rate of the observation group was 95.00%higher than that of the control group(77.50%),and the difference was statistically significant(χ^(2)=5.164,P<0.05).The time for symptomatic relief of the observation group was significantly shorter than that of the control group,and the difference was statistically significant(P<0.05).The levels of serum inflammatory factor indexes,such as serum C-reactive protein,interleukin-6,interleukin-17,were lower than those before the treatment,and the observation group was lower than the control group,and the differences were statistically significant(all P<0.05).The level of interleukin-10 was higher than that before treatment,and the observation group was higher than the control group,and the differences were statistically significant(both P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion For children with macrolide resistant MPP,doxycycline treatment has obvious effect,can promote the relief of symptoms in children,reduce the level of inflammatory factors,and has good safety.
