摘要
目的 探讨以团队为基础的教学法(team-based learning,TBL)在外科住院医师规范化培训(简称“住培”)学员腹腔镜手术基本操作的临床教学中的应用价值。方法 选择2021年7月—2022年12月于海军军医大学第二附属医院参加外科住培的84名学员为研究对象,随机将学员分为观察组和对照组各42名。比较2组间理论成绩、各项基本操作成绩、教学满意度评分和团队协作能力、TBL教学效果。结果 观察组学员理论测试成绩和教学满意度评价[(90.30±4.45)分、(8.54±0.83)分]均高于对照组[(86.72±6.27)分、(7.71±0.72)分],差异有统计学意义(P <0.05);观察组扶镜、剪切、分离、缝合打结等基本操作测试成绩均优于对照组,差异有统计学意义(P <0.05)。观察组学员运豆20粒所需时间[(9.03±1.57)min]短于对照组[(11.21±1.95)min],10 min内运豆总数[(22.19±2.58)粒]高于对照组[(17.84±3.60)粒],差异有统计学意义(P <0.05)。42名观察组学员中对TBL教学法普遍评价较高,均认为其可促进提高解决问题、团队合作精神和资料检索的能力。结论 将TBL教学法应用于普外科腹腔镜手术基本操作的教学有一定优势,可提高理论成绩、各项基本操作成绩、教学满意度评分和团队协作能力、TBL教学效果,值得逐步探索,但应因材施教,尊重学员个人学习习惯。
Objective To explore the application value of team-based learning(TBL)in clinical teaching of basic operation of laparoscopic gastrointestinal tumor surgery for the standardized residents training(short for residential trainee)in hospital.Methods A total of 84 residential trainees who participated in surgical residency training at the Second Affiliated Hospital of Naval Medical University from July 2021 to December 2022 were selected as the research subjects.They were randomly divided into an observation group and a control group,with 42 students in each group.The theoretical scores,basic operational scores,teaching satisfaction scores,team collaboration ability,and TBL teaching effectiveness between two groups were compared.Results The theoretical test scores and teaching satisfaction evaluations of the observation group students[(90.30±4.45)scores,(8.54±0.83)scores]were higher than those of the control group[(86.72±6.27)scores,(7.71±0.72)scores],with statistical differences(P<0.05).The observation group showed better results in basic operation tests such as assisting the mirror,cutting,separating,sewing and knotting than the control group,with statistical differences(P<0.05).The time required for the observation group to transport 20 beans[(9.03±1.57)minutes]was shorter than that of the control group[(11.21±1.95)minutes],and the total number of beans transported within 10 minutes[(22.19±2.58)beans]was higher than that of the control group[(17.84±3.60)beans],with a statistically significant difference(P<0.05).Among the 42 students in observation group,the TBL teaching method was generally highly praised,and they all believed that it could promote the improvement of problem-solving,teamwork spirit,and data retrieval abilities.Conclusion The application of TBL teaching method in the teaching of basic operations in laparoscopic general surgery has certain advantages,which can improve theoretical scores, various basic operation scores, teaching satisfaction scores, team collaboration ability, and TBL teaching effectiveness. It is worth gradually exploring, but teaching should be tailored to students and respect their individual learning habits.
