同轴活检技术应用于CT引导经皮肺活检对不明原因肺部病变的临床意义

Clinical significance of coaxial biopsy technique in CT guided percutaneous pulmonary biopsy in patients with unexplained pulmonary lesions

摘要 目的探讨同轴活检技术应用于CT引导经皮肺活检对不明原因肺部病变的临床意义。方法选取2015年5月至2019年5月清远市人民医院收治的306例不明原因肺部阴影患者作为研究对象,按照不同活检方式分为实验组(n=154)与对照组(n=152)。实验组采用同轴活检技术,对照组采用单纯针刺活检技术。比较两组手术成功率、并发症发生率、成功手术时间和成功手术接受辐射剂量。结果实验组小结节病灶手术成功率为98.3%,高于对照组的84.5%,差异有统计学意义(P<0.05);实验组深部病灶手术成功率为98.9%,高于对照组的90.7%,差异有统计学意义(P<0.05);实验组气胸、咯血发生率均低于对照组,差异有统计学意义(P<0.05);实验组成功手术时间短于对照组,成功手术接受辐射剂量少于对照组,差异有统计学意义(P<0.05)。结论同轴活检技术应用于CT引导经皮肺活检中对疑难穿刺病灶(小病灶和深部病灶)的手术成功率高于单纯针刺活检技术,可降低气胸和咯血发生率,减少患者接受的辐射剂量,缩短手术时间,且该项技术操作简单,成本低廉,只需在传统的单纯针刺活检技术的基础上附加同轴技术即可,值得临床推广应用。 Objective To explore the clinical significance of coaxial biopsy technique in CT guided percutaneous pulmonary biopsy in patients with unexplained pulmonary lesions.Methods 306 patients with unexplained pulmonary shadow treated in Qingyuan People's Hospital from May 2015 to May 2019 were selected as the study subjects,they were divided into the experimental group(n=154)and the control group(n=152)according to different biopsy methods,The experimental group underwent coaxial biopsy technique,and the control group underwent simple needle acupuncture biopsy technique.The operation success rate,the incidence of complications,the successful operation time and the radiation dose were compared between the two groups.Results The success rate of operation on small nodules in the experimental group was 98.3%,which was higher than 84.5%in the control group,and the difference was significant(P<0.05).The success rate of operation ondeep lesions in the experimental group was 98.9%,which was higher than 90.7%in the control group,and the difference was statistically significant(P<0.05).The incidence of complications of pneumothorax and haemoptysis in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).The successful operation time in the experimental group was shorter than that in the control group,and radiation dose received after successful operation in the experimental group were less than those in the control group,the differences were statistically significant(P<0.05).Conclusion The success rate of coaxial biopsy technique in CT guided percutaneous pulmonary biopsy for difficult puncture lesion(small lesions and deep lesions)is higher than the simple needle acupuncture biopsy technique,can reduce the incidence of pneumothorax and hemoptysis,reduce the radiation doses of patients,save the operation time,the technique is simple in operation and low in cost,and only needs to add coaxial technology on the basis of the traditional simple needle biopsy technology,which is worthy of clinical promotion and application.
