腹腔镜阑尾切除术对老年阑尾炎患者血清炎症指标及围手术期指标的影响

Effects of laparoscopic appendectomy on serum inflammatory markers and perioperative markers in elderly patients with appendicitis

目的探究腹腔镜阑尾切除术对老年阑尾炎患者血清炎症指标及围术期指标的影响。方法选取2015年1月至2021年1月吉安市中医院收治的80例老年阑尾炎患者作为研究对象,按照随机数字表法分为对照组与观察组,各40例。对照组行常规开腹阑尾切除术,观察组行腹腔镜下阑尾切除术。比较两组围手术期指标、炎症因子及术后并发症发生情况。结果观察组手术时间、首次排气时间、首次下床时间、术后住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后,观察组白细胞介素-6、肿瘤坏死因子-α及C反应蛋白水平均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为12.50%,低于对照组的32.50%,差异有统计学意义(P<0.05)。结论腹腔镜阑尾切除术可保证老年阑尾炎患者手术效果,减轻机体炎症反应,减少术后并发症发生,值得临床推广。

Objective To explore the effects of laparoscopic appendectomy on serum inflammatory markers and perioperative markers in elderly patients with appendicitis.Methods 80 elderly patients with appendicitis admitted to Ji'an City Traditional Chinese Medicine Hospital from January 2015 to January 2021 were selected as the research subjects,they were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The control group underwent routine open appendectomy,while the observation group underwent laparoscopic appendectomy.The perioperative indicators,inflammatory factors,and postoperative complications were compared between the two groups.Results The surgical time,first exhaust time,first time out of bed,and postoperative hospitalization days in the observation group were shorter than those in the control group,intraoperative bleeding volume was less than that in the control group,and the differences were statistically significant(P<0.05).After surgery,the levels of interleukin-6 and tumor necrosis factor-αand C-reactive protein in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications of the observation group was 12.50%,which was lower than 32.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic appendectomy can ensure the surgical effect of elderly patients with appendicitis,reduce the body's inflammatory response,and reduce the occurrence of postoperative complications,which is worthy of clinical promotion.
