振动排痰仪与密闭式排痰仪在慢性阻塞性肺疾病患者中的排痰效果及对肺功能的影响

Effect of Vibrating Expectoration Instrument and Closed Expectoration Instrument on Expectoration in Patients with Chronic Obstructive Pulmonary Disease and Their Influence on Lung Function

目的探讨振动排痰仪和密闭式排痰仪在COPD患者中的排痰效果及对肺功能的影响。方法选择南京同仁医院老年科2021年3月-2023年3月收治的50例COPD患者,随机分为两组,观察组使用振动排痰仪治疗,对照组使用密闭式排痰仪治疗。比较两组排痰量、肺功能、血气分析及生活质量。结果振动排痰组日排痰量明显大于密闭式排痰组,差异有统计学意义(P<0.05)。治疗后,振动排痰组FEV_(1)、FEV_(1)/FVC均明显大于密闭式排痰组,PaO_(2)、SaO_(2)均明显高于密闭式排痰组,PaCO_(2)低于密闭式排痰组,CRQ总分、呼吸困难、疲劳、情绪、掌控感评分均明显高于密闭式排痰组,差异有统计学意义(P<0.05)。结论振动排痰仪可有效促进COPD患者排痰,改善患者肺功能与气体交换功能,提高患者生活质量。

Objective:To Objective to investigate the expectoration effect of vibrating expectoration instrument and closed expectoration instrument in COPD patients and their influence on lung function.Methods:Fifty COPD patients admitted to the Geriatrics Department of Nanjing Tongren Hospital from March 2021 to March 2023 were randomly divided into two groups.The observation group was treated with vibrating expectoration instrument,while the control group was treated with closed expectoration instrument.The sputum output,lung function,blood gas analysis and quality of life were compared between the two groups.Results:The daily expectoration volume of vibration expectoration group was significantly higher than that of closed expectoration group,and the difference was statistically significant(P<0.05).After treatment,FEV_(1)、FEV_(1)/FVC in vibration expectoration group were significantly higher than those in closed expectoration group,PaO_(2)and SaO_(2)were significantly higher than those in closed expectoration group,PaCO_(2)was lower than that in closed expectoration group,and the total score of CRQ,dyspnea、fatigue、emotion and sense of control were significantly higher than those in closed expectoration group,with statistical significance(P<0.05).Conclusion:Vibrating expectoration instrument can effectively promote expectoration of COPD patients,improve their lung function and gas exchange function,and improve their quality of life.
