摘要
目的分析手法复位+经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩性骨折(OVCF)的临床疗效。方法以2021年10月至2022年10月东阳市横店医院骨科收治的的55例OVCF患者为研究对象,按照随机数字表法分为两组,均接受手法复位治疗,PKP组27例,接受经皮椎体球囊后凸成形术(PKP)治疗;PVP组28例,接受PVP治疗。对比两组手术一般情况、疼痛情况、腰椎功能及并发症发生率。结果PVP组手术时间短于PKP组,透视次数及治疗费用均少于PKP组,差异有统计学意义(P<0.05);两组术中注入骨水泥量对比,差异无统计学意义(P>0.05)。术前,两组VAS及ODI评分对比,差异无统计学意义(P>0.05);术后1 d及术后3个月,两组患者VAS和ODI评分均较术前下降,但组间对比,差异无统计学意义(P>0.05)。两组并发症发生率对比,差异无统计学意义(P>0.05)。结论手法复位联合PKP或PVP均可减轻OVCF患者疼痛,改善其腰椎功能,但PVP获得的经济效益更高,且不增加并发症发生率。
Objective:To analyze the clinical effect of manual reduction and percutaneous vertebroplasty(PVP)in the treatment of osteoporotic vertebral compression fracture(OVCF).Methods:From October 2021 to October 2022,55 patients with OVCF admitted to the Department of Orthopaedics,Hengdian Hospital,Dongyang City were divided into two groups according to the random number table method,all of them received manual reduction treatment,and 27 patients in PKP group received PKP treatment.28 cases in PVP group received PVP treatment.The general situation of operation,pain,lumbar function and incidence of complications were compared between the two groups.Results:The operation time in PVP group was shorter than that in PKP group,and the times of fluoroscopy and treatment cost were less than those in PKP group,with statistical significance(P<0.05).There was no significant difference in the amount of injected bone cement between the two groups(P>0.05).Before operation,there was no significant difference in VAS and ODI scores between the two groups(P>0.05).One day after operation and three months after operation,the scores of VAS and ODI in both groups decreased compared with those before operation,but there was no statistical difference between the two groups(P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Manual reduction combined with PKP or PVP can relieve the pain of patients with OVCF and improve their lumbar function,but PVP has higher economic benefits and does not increase the incidence of complications.
