1例肝硬化失代偿期并发下肢深静脉血栓患者抗凝治疗分析

Analysis of anticoagulant therapy for a patient with decompensated liver cirrhosis complicated with deep venous thrombosis

目的分析1例肝硬化失代偿期并发下肢深静脉血栓病例的治疗过程,探讨肝病患者抗凝药物选择的合理性,为临床合理用药提供参考。方法临床药师依据患者自身疾病特点及实验室检查指标评估血栓及出血风险,查阅相关文献,为肝硬化合并下肢深静脉血栓患者选择合适的抗凝方案,并对治疗过程全程监护。结果通过临床药师的干预,患者肝功能及凝血功能较前好转,病情得到缓解并继续治疗,出院随访1个月未发生血栓复发、出血等不良事件。结论临床药师协助医师制定个体化的抗凝治疗方案,为临床安全、有效用药提供保障,也为肝病患者抗凝治疗提供一定的指导。

Objective To analyze the treatment for a case of decompensated liver cirrhosis complicated with deep venous thrombosis,and to explore the rationality of the choice of anticoagulant drugs in patients with liver disease,so as to provide references for clinical rational drug use.Methods Clinical pharmacists assessed the risk of thrombosis and bleeding according to the patient's own disease characteristics and laboratory test indicators,reviewed the relevant literature,and selected the appropriate anticoagulation treatment for patients with liver cirrhosis complicated with deep venous thrombosis,and monitor the whole process of treatment.Results Through the intervention of clinical pharmacists,the liver function and coagulation function of the patient were improved,the condition was relieved and the treatment was continued.No adverse events such as thrombosis recurrence and bleeding occurred during the follow-up of one month after discharge.Conclusion In this case,the clinical pharmacist assisted physicians to formulate individualized anticoagulant therapy plan,which provides guarantee for safe and effective clinical drug use,and also provides certain guidance for anticoagulant therapy for patients with liver disease.
