基于MRT2WI影像组学诊断早期糖尿病肾病的应用价值研究

The applied value of MRT2WI imaging histology in the diagnosis of early diabetes nephropathy

目的探讨基于MRT2WI影像组学诊断早期糖尿病肾病的诊断价值。方法选取2021年1月—2023年1月本院收治的105例疑似糖尿病肾病患者为对象,对患者进行基于MRT2WI影像组学检查和生化检查,并以肾脏病理活检为金标准,评估对比MRT2WI影像组学诊断、生化检查诊断糖尿病肾病的阳性率,及与金标准诊断结果的一致性,并对检测方法的诊断效能进行统计对比。并对MRT2WI影像组学诊断与金标准诊断对糖尿病肾病的分期进行统计对比。结果经肾脏病理活检,105例患者中糖尿病肾病阳性患者91例、阴性14例,MRT2WI影像组学诊断检查糖尿病肾病阳性87例、阴性18例,生化检查阳性76例、阴性29例。经Kappa一致性检验,MRT2WI影像组学诊断结果与金标准有高度一致性(Kappa值=0.897),生化检查与金标准结果一致性一般(Kappa值=0.741)。在诊断效能指标对比中,MRT2WI影像组学诊断诊断的灵敏度、特异度、阳性预测值、阴性预测值、准确度均高于生化检查(P<0.05)。在糖尿病肾病分期诊断中,MRT2WI影像组学诊断与金标准方法结果差异无统计学意义(P>0.05)。结论基于MRT2WI影像组学诊断早期糖尿病肾病的应用价值良好,能够提高早期糖尿病肾病的诊断正确率,有较高的诊断效能,可在临床推荐使用。

Objective To study the diagnostic value of early diabetes nephropathy based on MRT2WI imaging.Methods 105 patients with suspected diabetes nephropathy admitted to our hospital from January 2021 to January 2023 were selected as the subjects,and MRT2WI imaging histology and biochemical examination were performed on the patients.With renal pathological biopsy as the gold standard,the positive rate of MRT2WI imaging histology diagnosis and biochemical examination in the diagnosis of diabetes nephropathy were evaluated and compared,and the consistency with the gold standard diagnosis results was also analyzed.The staging of diabetes nephropathy was statistically compared between MRT2WI imaging histology diagnosis and gold standard diagnosis.Results Through renal biopsy,91 cases were positive and 14 cases were negative in diabetes nephropathy,87 cases were positive and 18 cases were negative in MRT2WI imaging group,76 cases were positive and 29 cases were negative in biochemical examination.After the Kappa consistency test,the imaging omics diagnosis results of MRT2WI showed high consistency with the gold standard(Kappa value=0.897),while the consistency between the biochemical examination and the gold standard results was average(Kappa value=0.741).In the comparison of diagnostic efficacy indicators,the sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of MRT2WI imaging omics diagnosis were higher than those of biochemical tests(P<0.05).In the staging diagnosis of diabetes nephropathy,there was no statistical difference between the results of MRT2WI imaging and the gold standard method(P>0.05).Conclusion The application value of early diabetes nephropathy diagnosis based on MRT2WI imageomics is good,which can improve the diagnostic accuracy of early diabetes nephropathy,and has a high diagnostic efficiency,and can be recommended in clinical use.
