云南省2021年国家卫生城市(县城)暗访结果分析

Analysis on the results of check unannounced to the national health cities(counties)in Yunnan Province in 2021

目的分析云南省2021年55个国家卫生城市(县城)的暗访检查结果,为国家卫生城市(县城)的巩固和提升提供依据。方法采用全国统一的国家卫生城市暗访评价表,集中组队对云南省55个国家卫生城市(县城)进行暗访检查,评价结果采用0~10分的星级评分。采用均数、占比等对指标进行描述,采用χ^(2)检验、秩检验和偏相关分析进行组间比较和相关性分析。结果55个市县评价得分平均为7.90(7.90±0.24)分,其中群众满意度项评价得分最高(8.64分),城中村及城乡结合部卫生项评价得分最低(7.38分);城市的评分整体比县城高(χ^(2)=26.148,P<0.001),命名年数长的比命名年数短的市县评分高(χ^(2)=12.830,P=0.002),长效机制得分高的比长效机制得分低的市县评分高(χ^(2)=6.336,P=0.042),人均国内生产总值(gross domestic product,GDP)高的比人均GDP低的市县评分高(χ^(2)=10.263,P=0.006)。结论2021年云南省55个市县国家卫生城市(县城)暗访评分结果有明显差距,城市评分较县城高,命名时间长、长效机制做得好、人均GDP高的县市评分高。城中村及城乡结合部卫生、病媒生物预防控制、农贸市场卫生等项目是评价中的薄弱环节。要进一步加强长效管理,强化监督和健康影响评价,向健康城市迈进。

Objective To analyze the results of check unannounced to 55 national health cities(counties)in Yunnan Province in 2021,to provide a basis for the consolidation and promotion of national health cities(counties).Methods Using the nationally unified evaluation form of secret visits to national health cities,a centralized team was formed to conduct secret visits to 55 national health cities(counties),and the results were scored with a star score arranged 0-10.The indexes were described by means and proportion,compared and correlated between groups by Chi-square test,rank test,and partial correlation analysis.Results The average score of 55 cities and counties was 7.90(7.90±0.24),the evaluation score of mass satisfaction item was the highest(8.64),and the evaluation score of health in urban villages and urban-rural fringe item was the lowest(7.38).The overall scores of cities were higher than those of counties(χ^(2)=26.148,P<0.001),the score of the city and county with longer named years were higher than of the shorter named years(χ^(2)=12.830,P=0.002),the scores of the city and county with higher scores of long-term mechanism were higher than of the lower scores of long-term performance mechanism(χ^(2)=6.336,P=0.042),and scores of the city and county with higher per capita GDP were higher than of the lower per capita GDP(χ^(2)=10.263,P=0.006).Conclusion In 2021,there is a significant gap in the scores of secret visits to 55 cities and counties in Yunnan Province,and the overall scoring results of cities are higher than counties,the scores of cities with long named years,good long-term mechanisms,and high per capita GDP were high.Health projects in urban villages and urban-rural fringe,vector biological prevention and control,and farmers′market health were weak links in the evaluation.It is necessary to further strengthen long-term management,strengthen supervision,and health impact assessment,and move towards a healthy city.
