摘要
目的:探讨慢性阻塞性肺疾病(CDPD)稳定期患者血清纤维蛋白原(Fbg)、免疫球蛋白E(IgE)及呼出气一氧化氮(FENO)水平变化,分析其与疾病严重程度与肺功能的关系。方法:选取本院2020年8月-2023年11月60例COPD稳定期患者纳入COPD组(轻度23例、中度22例、重度15例),另选60例健康体检者作为对照组,检测两组肺功能指标第1秒用力呼气容积实测与预计值比值(FEV1%)、呼气峰流速(PEF)、最大呼气中段流量(MMF)水平,对比两组血清Fbg、IgE水平及FeNO水平差异,采用Spearman相关性分析其与疾病严重程度的关系,通过Pearson相关性分析COPD组患者肺功能指标与血清Fbg、IgE水平及FeNO水平的相关性。结果:COPD组肺功能指标FEV1%、MMF、PEF水平低于对照组(P<0.05);COPD组血清Fbg、IgE及FeNO水平高于对照组(P<0.05);重度患者血清Fbg、IgE及FeNO水平均高于中度组,中度组高于轻度组(P<0.05);Spearman相关性分析结果显示Fbg、IgE及FeNO水平分别与疾病严重程度呈现正相关(P<0.05);Pearson相关性分析结果显示Fbg、IgE及FeNO水平分别与肺功能指标FEV1%、MMF、PEF水平呈现负相关(P<0.05)。结论:COPD稳定期患者血清Fbg、IgE呈现高表达,FeNO水平升高,且其水平可在一定程度上反映患者疾病严重程度和肺功能水平。
Objective:To explore the changes of serum fibrinogen(Fbg),immunoglobulin E(IgE)and fractional exhaled nitric oxide(FENO)in patients with stable chronic obstructive pulmonary disease(CDPD),and to analyze their relationship with disease severity and lung function.Methods:A total of 60 patients with stable COPD(23 mild cases,22 moderate cases,15 severe cases)and 60 healthy controls in the hospital were enrolled as COPD group and control group between August 2020 and November 2023,respectively.The lung function indexes[forced expiratory volume in one second as a percentage of predicted value(FEV1%),peak expiratory flow(PEF),maximum mid-expiratory flow(MMF)]in the two groups were detected.The differences in levels of serum Fbg,IgE and FeNO between the two groups were compared,and the relationship between the three indexes and disease severity was analyzed by Spearman correlation analysis.The correlation between lung function indexes and serum Fbg,IgE,FeNO was analyzed by Pearson correlation analysis.Results:FEV1%,MMF and PEF in COPD group were lower than those in control group(P<0.05),while levels of serum Fbg,IgE and FeNO were higher than those in control group(P<0.05).The levels of serum Fbg,IgE and FeNO were gradually decreased in severe group,moderate group and mild group(P<0.05).The results of Spearman correlation analysis showed that levels of Fbg,IgE and FeNO were positively correlated with disease severity(P<0.05).The results of Pearson correlation analysis showed that Fbg,IgE and FeNO were negatively correlated with FEV1%,MMF and PEF(P<0.05).Conclusion:The levels of serum Fbg,IgE and FeNO are increased in patients with stable COPD,which all can reflect disease severity and lung function to a certain extent.
