身痛逐瘀汤联合渐进式牵引治疗气滞血瘀型腰椎间盘突出症临床观察

Shentong Zhuyu Decoction Combined with Progressive Traction in the Treatment of Lumbar Disc Herniation of Qi Stagnation and Blood Stasis Type

摘要 目的探讨身痛逐瘀汤加减联合渐进式牵引在气滞血瘀型腰椎间盘突出症(LDH)患者康复中的临床效果。方法选取2021年5月—2022年4月宜春市中医院收治的气滞血瘀型LDH患者66例,随机分为对照组与治疗组,每组33例。两组均予渐进式牵引治疗,对照组加用地塞米松磷酸钠注射液,治疗组加用身痛逐瘀汤加减,观察两组治疗前后视觉模拟量表(VAS)评分、日本骨科协会(JOA)下腰痛评分及临床疗效。结果治疗后,两组VAS评分均明显低于治疗前,且治疗组明显低于对照组(P<0.05);两组JOA评分均明显高于治疗前,且治疗1个月后治疗组JOA评分明显高于对照组(P<0.05)。治疗组总有效率为96.97%(32/33),明显高于对照组的81.82%(27/33)(P<0.05)。结论身痛逐瘀汤加减联合渐进式牵引能改善气滞血瘀型LDH患者的临床症状及腰椎功能,提高治疗效果,且安全性良好。 Objective To investigate the clinical effect of Shentong Zhuyu decoction combined with progressive traction in the rehabilitation of lumbar disc herniation(LDH)of qi stagnation and blood stasis type.Methods A total of 66 patients with LDH of qi stagnation and blood stasis type admitted to Yichun Hospital of Traditional Chinese Medicine from May 2021 to April 2022 were selected as the study objects and randomly divided into control group and treatment group,with 33 cases in each group.The control group was given progressive traction therapy and dexamethasone sodium phosphate injection treatment,and the treatment group was given progressive traction therapy and Shentong Zhuyu decoction.The improvement of VAS and JOA scores of the two groups before and after treatment was observed,and systematic analysis was made.Results After treatment,VAS scores in both groups were significantly lower than before treatment,and the treatment group was significantly lower than the control group(P<0.05).JOA scores in both groups were significantly higher than before treatment,and JOA scores in the treatment group were significantly higher than those in the control group after 1 month of treatment(P<0.05).The total effective rate of treatment group was 96.97%(32/33),which was significantly higher than that of control group(81.82%,27/33)(P<0.05).Conclusion Shentong Zhuyu decoction combined with progressive traction can improve the clinical symptoms and lumbar function of patients with LDH of qi stagnation and blood stasis type,and improve the therapeutic effect with good safety.
