痛风方对急性痛风血清IL-1β、IL-6、TNF-ɑ的影响与湿热浊瘀互结病机的构建

Influence of Tongfeng Prescription on Serum IL 1β,IL 6 and TNFαof Acute Gout and Construction of Pathogenesis of Intermingled Dampness-heat heat,Turbidity and Blood Stasis

目的基于痛风方对急性痛风血清炎症因子水平的影响,探析痛风湿热浊瘀互结病机理论构建。方法将2018年1月—2022年10月江西省中医院风湿科的35例急性痛风患者作为研究对象,采用痛风方进行治疗,疗程2~4周。比较患者治疗前后血清白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平变化以及治疗前三者在不同证候中的分布特点。结果治疗后,IL-6、TNF-α水平较治疗前降低,差异均有统计学意义(P<0.05);IL-1β水平与治疗前比较,差异无统计学意义(P>0.05)。治疗前,IL-1β、IL-6、TNF-α水平在湿热蕴结、湿热瘀结兼肝肾气虚证候组分布比较,差异均无统计学意义(P>0.05)。结论自拟痛风方对IL-6和TNF-α有较好的抑制作用。此研究在既往研究基础上,初步奠定痛风湿热浊瘀互结核心病机理论。

Objective To explore the theoretical construction of pathogenesis of gout with damp-heat,turbidity and blood stasis based on the influence of Tongfeng prescription on the levels of serum IL-1β,IL-6 and TNF-αin acute gout.Methods From January 2018 to October 2022,35 patients with acute gout in Jiangxi Provincial Hospital of Traditional Chinese Medicine were selected as the research objects.Tongfeng prescription was used for treatment for 2-4 weeks.The levels of serum interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)before and after treatment were compared,as well as the levels of IL-1β,IL-6 and TNF-αbefore treatment.Results The levels of IL-6 and TNF-αafter treatment were significantly lower than those before treatment(P<0.05).The level of IL-1βafter treatment was not significantly different from that before treatment(P>0.05).Before treatment,the levels of IL-1β,IL-6 and TNF-αwere different in the groups of damp-heat accumulation,damp-heat stasis and liver-kidney qi deficiency syndrome,but there was no significant difference(P>0.05).Conclusion On the basis of previous studies,this study preliminarily established the theory and construction law of gout,dampness-heat,turbid blood stasis.
