摘要
目的分析早期乳腺癌诊断中应用乳腺彩超的临床价值。方法选取2020年1月—2022年12月丰县人民医院收治的58例疑似乳腺癌患者为研究对象,入院患者均行乳腺X线、乳腺彩超检查,以病理检查为金标准,对比不同检查方案的检出结果、诊断效能,以及不同检查方案对超微细钙化、血流信号的检出情况。结果病理诊断良性(阴性)36例、恶性(阳性)22例;乳腺彩超检出阴性36例、阳性22例;乳腺X线检出阴性35例、阳性23例。乳腺彩超诊断灵敏度、特异度、准确度分别为95.45%、97.22%、96.55%,均高于乳腺X线的68.18%、77.78%、74.14%,差异有统计学意义(P均<0.05)。病理检出超微细钙化24例、血流信号18例;乳腺彩超超微细钙化诊断符合率低于X线,血流信号诊断符合率高于X线,差异有统计学意义(P均<0.05)。结论将乳腺彩超用于早期乳腺癌诊断效果显著,诊断效能较高,能够为疾病诊断鉴别提供科学参考;但乳腺彩超在钙化征象方面敏感度较低,而乳腺X线则较高,故必要下可二者结合使用,优势互补,提高诊断准确性。
Objective To analyze the clinical value of the application of breast color ultrasound in the diagnosis of early breast cancer(BC).Methods Fifty-eight suspected BC patients admitted to Fengxian People's Hospital from January 2020 to December 2022 were conveniently selected as the study objects,and all the admitted patients underwent mammography and breast color ultrasound,and the detection results and diagnostic efficacy of different inspection schemes were compared with those of different inspection schemes using pathology as the gold standard,as well as the detection of ultrafine calcification and blood flow signals by different inspection schemes.Results Pathology diagnosed 36 cases of benign(negative)and 22 cases of malignant(positive),breast color ultrasound detected 36 cases of negative and 22 cases of positive,mammography detected 35 cases of negative and 23 cases of positive.The diagnostic sensitivity,specificity,and accuracy of breast color ultrasound were 95.45%,97.22%,and 96.55%,respectively,which were significantly higher than those of mammography(68.18%,77.78%),and 74.14%,and the differences were statistically significant(all P<0.05).Pathology detected 24 cases of ultrafine calcification and 18 cases of blood flow signals,the diagnostic compliance rate of ultrafine calcification of breast ultrasound was lower than that of mammography,and the diagnostic compliance rate of blood flow signals was higher than that of mammography,and the differences were statistically significant(both P<0.05).Conclusion Breast color ultrasound is used in early BC diagnosis with remarkable effect and high diagnostic efficacy,which can provide scientific reference for disease diagnosis and identification.However,the sensitivity of breast color ultrasound in the calcification signs is lower,while mammography is higher,so the two can be combined if necessary to complement each other and improve the diagnostic accuracy.
