输尿管上段结石患者采用输尿管软镜下钬激光碎石取石术治疗的价值

Value of Holmium Laser Lithotripsy under Ureteral Soft Scope in the Treatment of Upper Ureteral Calculi

目的探讨输尿管软镜下钬激光碎石取石术治疗输尿管上段结石的效果。方法回顾性分析2021年6月—2023年11月黔东南州人民医院收治的100例输尿管上段结石患者的临床资料,根据治疗方法的不同分为对照组(50例,输尿管硬镜取石术)与研讨组(50例,输尿管软镜下钬激光碎石取石术)。比较两组手术情况、感染指标、疼痛情况、并发症情况、住院指标。结果与对照组相比,研讨组的手术时间更长、术中出血量更低,差异有统计学意义(P均<0.05)。研讨组的感染指标低于对照组,差异有统计学意义(P<0.05)。研讨组术后48、72 h的视觉模拟量表(Visual Analogue Scale,VAS)评分均低于对照组,差异有统计学意义(P均<0.05)。研讨组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。研讨组的下床时间、住院时间均短于对照组,差异有统计学意义(P均<0.05)。结论输尿管软镜下钬激光碎石取石术手术优势更为明显,能减轻患者术后炎性反应与疼痛症状,优化患者预后进程,并降低术后风险。

Objective To study the effect of holmium laser lithotripsy under ureteral soft scope on upper ureteral calculi.Methods The clinical data of 100 patients with upper ureteral calculi admitted to Qiandongnanzhou People's Hospital from June 2021 to November 2023 were retrospectively selected and divided into the reference group(50 cases,ureteroscopic lithotripsy)and the study group(50 cases,ureteroscopic holmium laser lithotripsy)according to different treatment methods.Surgical situation,infection indexes,pain situation,complications situation and hospitalization indexes were compared between the two groups.Results Compared with the reference group,the operation time was longer and the amount of blood loss was lower in the study group,and the differences were statistically significant(both P<0.05).The infection index of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).The visual Analogue Scale(VAS)score of the research group at 48,72 h after surgery was lower than that of the reference group,and the differences were statistically significant(both P<0.05).The total complication rate of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).The duration of getting out of bed and staying in hospital in the study group were shorter than those in the reference group,and the differences were statistically significant(both P<0.05).Conclusion Holmium laser lithotripsy under ureteroscope has more obvious advantages,which can reduce postoperative inflammatory response and pain symptoms,optimize the prognosis of patients,and reduce postoperative risk.
