桑蒺温胆汤联合西药对高血压急症心力衰竭患者心功能及生化指标的影响

Effect of Sangji Wendan Decoction Combined with Western Medicine on Cardiac Function and Biochemical Indicators in Patients with Acute Heart Failure Caused by Hypertension

目的观察桑蒺温胆汤联合西药对高血压急症心力衰竭(AHF)的疗效及对心功能、相关生化指标的影响。方法将102例高血压急症AHF患者随机分为对照组与观察组各51例。对照组应用西药治疗,观察组在西药治疗基础上联用桑蒺温胆汤治疗,治疗时间为7 d。评价中医证候评分变化,并检测24 h尿量、左室射血分数(LVEF)、左室舒张与收缩末内径(LVEDD、LVESD)、舒张压(DBP)、收缩压(SBP)、平均动脉压(MAP),检测血清心肌肌钙蛋白I(cTnI)、氨基末端B型利钠肽前体(NT-proBNP)水平。结果治疗7 d后,观察组的中医证候评分显著低于对照组(P<0.05)。两组治疗后血压(SBP、DBP、MAP)差异不显著(P<0.05),而观察组的24 h尿量、LVEF较对照组显著提升(P<0.05),LVEDD、LVESD、血清cTnI和NT-proBNP均较对照组显著降低(P<0.05)。观察组总有效率为96.08%,高于对照组的82.35%(P<0.05)。结论桑蒺温胆汤联合西药治疗高血压急症AHF能够改善临床症状,保护心功能,提高治疗效果。

Objective:To observe the efficacy of Sangji Wendan Decoction combined with western medicine for patients with acute heart failure(AHF)caused by hypertension and its impact on cardiac function and related biochemical indicators.Methods:A total of 102 patients with AHF caused by hypertension were randomly divided into two groups,51 cases in each group.The control group was treated with western medicine,and on this basis,the observation group was treated with Sangji Wendan Decoction.Both groups were treated for 7 days.The changes in traditional Chinese medicine symptom scores were evaluated,and 24-hour urine output,left ventricular ejection fraction(LVEF),left ventricular diastolic and end systolic internal meridian(LVEDD,LVESD),diastolic blood pressure(DBP),systolic blood pressure(SBP)mean arterial pressure(MAP)was monitored,and serum levels of cardiac troponin I(cTn I)and N-terminal B-type natriuretic peptide precursor(NT proBNP)were detected.Results:In the observation group,the decrease of traditional Chinese medicine symptom scores after 7 days of treatment was more significantly compared with that of the control group(P<0.05).There was no significant difference in the results between the two groups in terms of blood pressure(SBP,DBP,and MAP)after treatment(P>0.05),while the 24-hour urine output and LVEF of the observation group were significantly increased compared to the control group(P<0.05),the LVEDD,LVESD,serum cTn I,and NT-proBNP were significantly reduced compared with the control group(P<0.05).The total effective rate of the observation group was 96.08%,which was significantly higher than 82.35%of the control group(P<0.05).Conclusion:Sangji Wendan Decoction combined with Western medicine can improve clinical symptoms,protect heart function,and enhance treatment efficacy in the treatment of patients with AHF caused by hypertension.
