宫腔镜下子宫内膜息肉电切术对育龄期患者妊娠结局的影响分析

Analysis of the influence of hysteroscopic electroresection of endometrial polyps on pregnancy outcomes in patients of childbearing age

目的 探讨宫腔镜下子宫内膜息肉电切术对育龄期患者妊娠结局及妊娠率的影响。方法 选取68例行宫腔镜手术的子宫内膜息肉育龄期患者,根据随机数字表法分组,每组34例。对照组应用宫腔镜下刮除术,试验组应用宫腔镜下电切术。比较两组临床指标(手术与住院用时、术中出血量)及术后并发症发生情况;随访1年,观察不良妊娠结局发生率、妊娠率。结果 试验组手术时间、住院时间均短于对照组,术中出血量少于对照组(P<0.05)。试验组不良妊娠结局发生率2.94%低于对照组的23.53%,妊娠率88.24%高于对照组的67.65%(P<0.05),术后并发症发生率5.88%低于对照组的29.41%(P<0.05)。结论 宫腔镜下电切术有助于降低子宫内膜息肉患者不良妊娠结局的风险,提升临床妊娠率,缩短手术与住院时间,减少术中出血量,并降低术后并发症发生率。

Objective To investigate the effect of hysteroscopic electro re section of endometrial polyps on pregnancy outcome and pregnancy rate in patients of childbearing age.Methods 68 patients of reproductive age with endometrial polyps undergoing hysteroscopic surgery were selected and divided into 34 patients in each group according to random number table method.The control group underwent hysteroscopic curettage and the experimental group underwent hysteroscopic electrotomy.The clinical indexes(duration of operation and hospitalization,intraoperative blood loss) and postoperative complications were compared between the two groups.The incidence of adverse pregnancy outcome and pregnancy rate were followed up for1 year.Results The operation time and hospitalization time of experimental group were shorter than that of control group,and the intraoperative blood loss was less than that of control group(P<0.05).The incidence of adverse pregnancy outcome in the experimental group was 2.94% lower than that in the control group(23.53%),and the pregnancy rate was 88.24% higher than that in the control group(67.65%)(P<0.05).The postoperative complication rate of experimental group was 5.88% lower than that of control group(29.41%)(P<0.05).Conclusion Hysteroscopic electroresection is helpful to reduce the risk of adverse pregnancy outcomes in patients with endometrial polyps,improve the clinical pregnancy rate,shorten the duration of surgery and hospital stay,reduce intraoperative blood loss,and reduce the incidence of postoperative complications.
