胸苷激酶1、细胞角质蛋白19片段抗原21-1在非小细胞肺癌中的表达及与患者临床特征和预后的关系

摘要 目的探讨胸苷激酶1(TK1)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)在非小细胞肺癌(NSCLC)中的表达及与患者临床特征和预后的关系。方法选取100例NSCLC患者,采用免疫组化法检测肿瘤组织及癌旁组织中TK1、CYFRA21-1表达情况,比较NSCLC患者肿瘤组织与癌旁组织中TK1、CYFRA21-1的表达情况,以及不同临床特征NSCLC患者肿瘤组织中TK1、CYFRA21-1的表达情况。根据预后情况将NSCLC患者分为预后不良组和预后良好组,采用Logistic回归分析NSCLC患者的预后影响因素。结果肿瘤组织中TK1、CYFRA21-1的阳性表达率均明显高于癌旁组织,差异均有统计学意义(P﹤0.01)。不同淋巴结转移情况NSCLC患者肿瘤组织中TK1、CYFRA21-1阳性表达率比较,差异均有统计学意义(P﹤0.05)。预后良好组(n=71)和预后不良组(n=29)患者淋巴结转移情况及TK1、CYFRA21-1表达情况比较,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,有淋巴结转移及TK1、CYFRA21-1阳性表达均是NSCLC患者预后不良的独立危险因素(P﹤0.05)。结论TK1、CYFRA21-1在NSCLC中的阳性表达率较高,有淋巴结转移及TK1、CYFRA21-1阳性表达均是NSCLC患者预后不良的危险因素。 Objective To investigate the expression of thymidine kinase 1(TK1)and cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)in non-small cell lung cancer(NSCLC)and their relationship with clinical features and prognosis.Method A total of 100 NSCLC patients were selected,and the expression of TK1 and CYFRA21-1 in tumor tissues and adjacent tissues were detected by immunohistochemical method.The expression of TK1 and CYFRA21-1 in tumor tissues and adjacent tissues,and NSCLC patients with different clinical features were compared.According to the prognosis,NSCLC patients were divided into poor prognosis group and good prognosis group.Logistic regression analysis was used to analyze the influencing factors on prognosis of NSCLC patients.Result The positive expression rates of TK1 and CYFRA21-1 in tumor tissues were significantly higher than those in adjacent tissues,and the differences were statistically significant(P<0.01).The positive expression rates of TK1 and CYFRA21-1 in NSCLC patients with different lymph node metastasis were compared,and the differences were statistically significant(P<0.05).Lymph node metastasis,expression of TK1 and CYFRA21-1 in good prognosis group(n=71)and poor prognosis group(n=29)were compared,and the differences were statistically significant(P<0.05).Lymph node metastasis,positive expression of TK1 and CYFRA21-1 were independent risk factors for poor prognosis in NSCLC patients(P<0.05).Conclusion The positive expression rates of TK1 and CYFRA21-1 in NSCLC are high,and lymph node metastasis,positive expression of TK1 and CYFRA21-1 are risk factors for poor prognosis in NSCLC patients.
