宣肺止嗽合剂联合阿奇霉素治疗儿童支原体肺炎的临床疗效

Effect of Xuanfei Zhisou Mixture Combined with Azithromycin in the Treatment of Mycoplasma Pneumonia in Children and Its Effect on the Repair of Bronchial Mucosal Injury

目的:探讨宣肺止嗽合剂联合阿奇霉素对支原体肺炎患儿的治疗效果。方法:选择2022年10月至2023年10月南昌大学第一附院医院高新医院收治的200例支原体肺炎患儿,随机分为两组,每组各100例。对照组单用阿奇霉素,观察组在对照组基础上联用宣肺止嗽合剂。比较两组患儿的支气管黏膜损伤率、肺功能指标、临床症状评分、炎症因子水平和不良反应情况。结果:观察组患儿的黏膜充血水肿率、黏膜滤泡样增生率、支气管开口红肿率和黏膜糜烂率均明显低于对照组,差异具有统计学意义(P<0.05);治疗后,两组患儿的肺功能相关指标[1 s用力呼气容积占用力肺活量比值(FEV1/FVC)、最大呼气中期流速(MMF)、最大肺活量(VCmax)、呼气峰值流量(PEF)、最大自主通气量(MVV)和FEV1%预期值]均升高,且观察组高于对照组,差异具有统计学意义(P<0.05);治疗后,两组患儿的咳嗽、食欲不振、喘憋和发热评分均降低,且观察组低于对照组,差异具有统计学意义(P<0.05);治疗后,两组患儿血清炎症因子水平均降低,且观察组患儿血清降钙素原(PCT)、基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF-α)和白细胞介素-8(IL-8)水平明显低于对照组,差异具有统计学意义(P<0.05);两组患儿的腹泻、皮疹、腹痛和恶心呕吐发生率比较,差异无统计学意义(P>0.05)。结论:宣肺止嗽合剂联合阿奇霉素治疗支原体肺炎患儿有显著的效果,能有效改善肺功能和修复支气管黏膜损伤。

Objective To explore the therapeutic effect of Xuanfei Zhisou mixture combined with azithromycin on children with mycoplasma pneumonia.Methods A total of 200 children with mycoplasma pneumonia admitted to Gaoxin Hospital of the First Affiliated Hospital of Nanchang University from October 2022 to October 2023 were selected and randomly divided into two groups,100 cases in each group.The control group was treated with azithromycin alone,and the observation group was treated with Xuanfei Zhisou mixture on the basis of the control group.The rate of bronchial mucosal injury,pulmonary function indicators,clinical symptom scores,levels of inflammatory factors,and adverse effects were compared between the two groups.Results The rates of mucosal congestion,edema,mucosal follicle-like hyperplasia,bronchial opening redness and mucosal erosion in the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the lung function related indexes[forced expiratory volume in first second/forced vital capacity(FEV1/FVC),maximum mid expiratory flow rate(MMF),maximum vital capacity(VCmax),peak expiratory flow(PEF),maximum voluntary ventilation(MVV)and FEV1%predictive value]in the two groups increased,and those in the observation group were higher than the control group,the differences were statistically significant(P<0.05).After treatment,the scores of cough,loss of appetite,wheezing,and fever were decreased in the two groups,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the serum levels of inflammatory factors decreased in both groups,and the serum levels of procalcitonin(PCT),matrix metalloproteinase-9(MMP-9),tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8)were significantly lower than those of the control group,with statistically significant differences(P<0.05).There is no statistically significant difference in the incidence of diarrhea,rash,abdominal pain,and nausea and vomiting between the two groups of children(P>0.05).Conclusion Xuanfei Zhisou mixture combined with azithromycin has a significant effect in the treatment of children with mycoplasma pneumonia,which can effectively improve lung function and repair bronchial mucosal injury.
