螺旋CT靶重建在独立性肺结节定性诊断中的应用价值

Application Value of Spiral CT Target Reconstruction in Qualitative Diagnosis of Independent Pulmonary Nodules

目的:探究螺旋计算机断层扫描(CT)靶重建在独立性肺结节(SPN)定性诊断中的应用价值。方法:选择2022年12月至2023年3月河南中医药大学第三附属医院收治的80例SPN患者开展研究,全部患者均行螺旋CT平扫及薄层靶重建检查,比较两种方式对SPN征象的检出率。并将病理检查结果作为金标准,比较两种方式对良恶性SPN的诊断价值。结果:经病理检查,80例SPN患者中良性、恶性SPN分别为32例(40.00%)、48例(60.00%)。螺旋CT靶重建对SPN毛刺征、分叶征、支气管气相征、空泡征、胸膜牵拉征、血管集束征的检出率均比螺旋CT平扫更高,差异均具有统计学意义(P<0.05)。螺旋CT靶重建对肺腺癌SPN微浸润腺癌、浸润腺癌诊断符合率均比螺旋CT平扫更高,差异均具有统计学意义(P<0.05)。螺旋CT靶重建对良恶性SPN诊断的准确度、灵敏度、特异度及阳性、阴性预测值比螺旋CT平扫的高,差异均具有统计学意义(P<0.05)。结论:螺旋CT靶重建在SPN定性诊断中的诊断价值较高,影像学征象清晰易辨。

Objective To explore the application value of spiral computed tomography(CT)target reconstruction in the qualitative diagnosis of independent pulmonary nodule(SPN).Methods A total of 80 patients with SPN admitted to the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from DecembeR2022 to March 2023 were selected forthis study.All patients underwent spiral CT plain scan and thin-layer target reconstruction examination,and the detection rates of SPN signs by the two methods were compared.The results of pathological examination were used as the gold standard to compare the diagnostic value of the two methods in benign and malignant SPN.Results Pathological examination showed that 32 cases(40.00%)of SPN were benign and 48 cases(60.00%)of malignant SPN.The detection rates of SPN burRsign,lobed sign,bronchial gas sign,vacuole sign,pleural stretch sign and vasculaRclusteRsign were higherin spiral CT target reconstruction than in spiral CT plain scan,and the differences were statistically significant(P<0.05).Target reconstruction of spiral CT had higherdiagnostic coincidence rate forSPN microinvasive adenocarcinoma and invasive adenocarcinoma than that of spiral CT plain scan,and the differences were statistically significant(P<0.05).The accuracy,sensitivity,specificity and positive and negative predictive values of spiral CT target reconstruction forbenign and malignant SPN were higherthan those of spiral CT plain scan,and the differences were statistically significant(P<0.05).Conclusion Target reconstruction of spiral CT has high diagnostic value in qualitative diagnosis of SPN,and the imaging signs are cleaRand easy to distinguish.
