表皮生长因子受体突变型与肺腺癌生物学效应影像特征相关性分析

摘要 目的:探究肺腺癌(LAD)患者表皮生长因子受体(EGFR)突变与CT特征与相对酸化率的相关性。方法:选取2016年1月—2023年7月于包头市肿瘤医院就诊的经病理证实为LAD的75例患者。所有受试者都接受了EGFR突变状态检测,并在治疗前行胸部CT检查,由两名医师评价肺部CT特征与相对酸化率,统计分析肺部CT特征、相对酸化率与EGFR突变状态的关系,选出有统计学差异的变量进行二元Logistic回归分析。结果:75例肺LAD者中,EGFR突变型45例,野生型30例,EGFR突变在女性患者中常见,胸部CT扫描常表现血管集束征、周围牵拉肺气肿、肿瘤最大径小、相对酸化率低。Logistic回归分析显示最大径、相对酸化率均是影响发生突变的独立保护因素。结论:在LAD患者中,血管集束征、周围牵拉肺气肿、最大径小、相对酸化率低的女性患者可能提示存在EGFR基因突变。 Objective To investigate the correlation between epidermal growth factor receptor(EGFR)mutations and CT features and relative acidification rate in lung adenocarcinoma patients.Methods 75 patients with lung adenocarcinoma confirmed by pathology in Baotou Cancer Hospital from January 2016 to July 2023 were collected.All subjects were tested for EGFR mutation status,and chest CT examination was performed before treatment.Two radiologists evaluated pulmonary CT characteristics and relative acidification rate,statistically analyzed the relationship between pulmonary CT characteristics,relative acidification rate and EGFR mutation state,and selected variables with statistical differences for binary Logistic regression score.Results Among 75 patients with lung adenocarcinoma,there were 45 EGFR mutant and 30 wild-type,EGFR mutation was common in female patients,and chest CT scans often showed vascular cluster sign,peripheral pulling emphysema,small tumor maximum diameter,and low relative acidification rate.Logistic regression analysis showed that the maximum diameter and the relative acidification rate were independent protective factors affecting the occurrence of mutation.Conclusion In patients with lung adenocarcinoma,vascular cluster sign,peripheral pulling emphysema,small maximum diameter,and low relative acidification rate in female patients may suggest the presence of EGFR gene mutation.
