高压氧治疗联合早期综合康复护理对缺氧缺血性脑病患儿神经功能及血清IGF-1、IL-6、NSE水平的影响

摘要 目的探讨高压氧治疗联合早期综合康复护理对缺氧缺血性脑病(HIE)患儿神经功能及血清胰岛素样生长因子-1(IGF-1)、白细胞介素-6(IL-6)、神经元特异性烯醇化酶(NSE)水平的影响。方法选取2019年2月至2022年2月临沂市人民医院神经外科收治的62例HIE患儿为研究对象,按照随机数字表法分为观察组和对照组,每组31例。2组患儿均接受常规治疗及高压氧治疗,对照组予以常规护理,观察组则予以早期综合康复护理。比较2组患儿的主要症状恢复时间、干预前后新生儿神经行为测定量表(NBNA)评分及血清IGF-1、IL-6、NSE水平差异。结果观察组患儿的意识恢复时间、反射恢复时间、肌张力恢复时间、可吸吮恢复时间均明显短于对照组,差异有统计学意义(P<0.001)。干预前,2组患儿的NBNA评分及血清IGF-1、IL-6、NSE水平比较差异无统计学意义(P>0.05);干预后,2组患儿的NBNA评分及血清IGF-1水平均高于干预前,且观察组高于对照组,而血清IL-6、NSE水平均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.001)。结论高压氧治疗联合早期综合康复护理可有效改善HIE患儿的神经功能,提高血清IGF-1水平,降低血清IL-6、NSE水平。 Objective To investigate the effects of hyperbaric oxygen therapy combined with early comprehensive rehabilitation nursing on the neurological function and serum levels of insulin-like growth factor-1(IGF-1),interleukin-6(IL-6),and neuron-specific enolase(NSE)in children with hypoxic-ischemic encephalopathy(HIE).Methods A total of 62 children with HIE admitted to the Neurosurgery Department of Linyi People’s Hospital from February 2019 to February 2022 were selected as the study objects,and were divided into observation group and control group according to the random number table method,with 31 cases in each group.On the basis of hyperbaric oxygen therapy,the control group received conventional nursing,and the observation group received early comprehensive rehabilitation nursing.The main symptoms'recovery time,neonatal behavioral neurological assessment(NBNA)score,and serum levels of IGF-1,IL-6,and NSE before and after intervention were compared between the two groups.Results The consciousness recovery time,reflex recovery time,muscle tension recovery time,and sucking recovery time in the observation group were significantly shorter than those in the control group,and all the differences were statistically significant(P<0.001).Before intervention,there was no statistically significant difference in NBNA scores and serum levels of IGF-1,IL-6,and NSE between two groups(P>0.05).After intervention,the NBNA score and serum IGF-1 level in two groups were higher than those before intervention,and those in the observation group were higher than those in the control group,while the serum IL-6 and NSE levels were lower than those before intervention,and those in the observation group were lower than those in the control group,with statistically significant differences(P<0.001).Conclusion Hyperbaric oxygen therapy combined with early comprehensive rehabilitation nursing can effectively improve the neurological function of children with HIE,increase the level of serum IGF-1,and reduce the levels of serum IL-6 and NSE.
