天麻钩藤饮加减联合常规西医治疗高血压伴眩晕患者的临床效果观察

Observation on the clinical effect of Modified Tianma Gouteng Decoction combined with conventional Western medicine in treating hypertension patients with dizziness

目的 探讨天麻钩藤饮加减联合常规西医治疗高血压伴眩晕的效果及对患者血流动力学、眩晕症状的影响。方法 选择呼和浩特市蒙医中医医院2020年1月至2023年1月收治的高血压伴眩晕患者80例,按照随机数表法分为对照组和观察组,每组各40例。对照组采用氨氯地平治疗,观察组在对照组的基础上联合天麻钩藤饮加减治疗,两组均用药2个月。比较两组临床疗效、中医症候积分、血流动力学、醛固酮(ALD)、血管紧张素Ⅱ(AngⅡ)水平及不良反应。结果 观察组总有效率高于对照组,差异有统计学意义(P <0.05)。治疗后,观察组中医症候积分、收缩压、舒张压、ALD、AngⅡ水平低于对照组,差异有统计学意义(P <0.05)。两组不良反应总发生率比较,差异无统计学意义(P> 0.05)。结论 对高血压伴眩晕患者实施天麻钩藤饮加减联合常规西药治疗可显著改善患者眩晕等症状,降低患者血压水平,调节肾素-血管紧张素-醛固酮系统,且用药的安全性良好。

Objective To explore the therapeutic effect of modified Tianma Gouteng Decoction combined with conventional Western medicine on hypertension with dizziness and its impact on hemodynamics and dizziness symptoms in patients.Methods A total of 80 hypertension patients with dizziness admitted to the Hohhot Mongolian Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were divided into the control group and the observation group using a random number table method,with 40 patients in each group.The control group was treated with amlodipine,and the observation group was treated on the basis of the control group by combining Modified Tianma Gouteng Decoction.Both groups were treated for 2 months.The clinical efficacy,traditional Chinese medicine symptom scores,hemodynamics,aldosterone(ALD),angiotensinⅡ(AngⅡ)levels,and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,with a statistically significant difference(P<0.05).After treatment,traditional Chinese medicine syndrome score,systolic blood pressure,diastolic blood pressure,ALD and AngⅡlevels in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The total incidence of adverse reactions were compared between the two groups,with no statistically significant difference(P>0.05).Conclusion Treating patients with hypertension and vertigo with Modified Tianma Guteng Decoction combined with conventional western medicine can significantly improve vertigo and other symptoms,reduce blood pressure,regulate renin-angiotensin-aldosterone system,and the safety of the drug is good.
