围产期哺乳顾问联合乳房保健干预对初产妇母乳喂养成功率及新生儿有效吸吮情况的影响

Effect of Perinatal Lactation Consultant Combined with Breast Healthcare Intervention on the Success Rate of Breastfeeding in Primiparas and Effective Sucking of Neonates

目的:探讨围产期哺乳顾问联合乳房保健干预对初产妇母乳喂养成功率及新生儿有效吸吮情况的影响,为提倡初产妇母乳喂养提供依据。方法:选取2019年1月—2022年6月在宜春市中医院分娩的92例初产妇作为研究对象,按照区组随机化分组法将其分为对照组和观察组,每组各46例。对照组实施常规护理干预,观察组在常规护理基础上实施围产期哺乳顾问联合乳房保健干预。比较两组初产妇母乳喂养状况满意度[母乳喂养评估量表(MBFES)评分]、母乳喂养成功率、乳胀发生率、新生儿有效吸吮及无效吸吮情况。结果:产后3个月,两组初产妇MBFES中各维度评分及总分均较产后1个月明显升高,且观察组高于同期对照组,差异均有统计学意义(t=2.965、2.987、3.978,P<0.05)。产后1个月和产后3个月,观察组初产妇母乳喂养成功率均高于对照组,差异均有统计学意义(χ^(2)=4.483、4.039,P<0.05);两组初产妇乳胀发生率比较,差异均无统计学意义(χ^(2)=0.448、0.261,P>0.05)。产后1个月和产后3个月,观察组初产妇新生儿有效吸吮次数均明显高于对照组,无效吸吮次数均明显低于对照组,差异均有统计学意义(t=4.538、4.334、3.868、9.482,P<0.05)。结论:对初产妇实施围产期哺乳顾问联合乳房保健干预,能够提高初产妇母乳喂养状况满意度和母乳喂养成功率,改善新生儿有效吸吮次数。

Objective:To explore the effect of perinatal lactation consultant combined with breast health intervention on the success rate of breast feeding in primiparas and the effective suckling of newborns,so as to provide basis for maternal breastfeeding.Methods:A total of 92 primiparas who delivered in Yichun Hospital of Traditional Chinese Medicine from January 2019 to June 2022 were selected.They were randomly divided into control group and observation group,with 46 cases in each group.Control group was given routine nursing intervention,while observation group was given nursing intervention in combination with breast health care.The breast-feeding satisfaction(MBFES),breast-feeding success rate,breast distension rate,effective and ineffective sucking of newborn were compared between the two groups.Results:Three months after delivery,the scores of all dimensions and total scores of MBFES in the two groups were significantly higher than those in the control group at the same time(t=2.965,2.987,3.978;P<0.05).The breast-feeding success rate of the observation group was higher than that of the control group 1 and 3 months after delivery(χ^(2)=4.483,4.039;P<0.05).There was no significant difference(χ^(2)=0.448,0.261;P>0.05).1 and 3 months after delivery,the effective sucking times in the observation group were significantly higher than those in the control group,while the ineffective sucking times in the observation group were significantly lower than those in the control group,with statistically significant difference(t=4.538,4.334,3.868,9.482;P<0.05).Conclusion:Perinatal lactation counsellors in conjunction with breast health interventions can increase the success rate of breastfeeding among primiparas and the number of effective suckling episodes in newborns,and increase maternal breastfeeding satisfaction.
