揿针治疗对中老年椎体融合术后患者恶心呕吐的效果观察

Observation on the Effect of Pressing Needle Therapy on Nausea and Vomiting in Middle-Aged and Elderly Patients After Vertebral Fusion Surgery

目的探讨椎体骨折中老年患者椎体融合术后应用揿针治疗降低术后恶心呕吐发生率的作用。方法选取福建医科大学附属第二医院2021年8月—2023年2月在全麻下行椎体融合术后携带静脉自控镇痛泵的中老年患者74例,通过随机数字表法分为2组。对照组出现恶心呕吐等胃部不适时,给予心理疏导、转移注意力,加强巡视观察,必要时根据医嘱,调整镇痛泵的使用方法或者使用药物止吐;观察组在对照组的基础上予术后即刻及术后12 h采用双侧合谷及内关揿埋针疗法。比较2组患者术后4、8、12、24 h恶心及呕吐严重程度及发生率情况。结果术后24 h内观察组患者控制有效率(81.08%)显著高于对照组(54.05%),差异有统计学意义(P<0.05)。观察组首次呕吐时间、呕吐持续时间显著短于对照组(P<0.05)。结论对全麻下行椎体融合手术的中老年患者实施穴位埋针治疗,可以更高效降低患者术后恶心呕吐发生率,并减轻其恶心呕吐程度,且较少出现不良反应。

Objective To explore the effect of pressing needle therapy on reducing the incidence of postoperative nausea and vomiting in middle-aged and elderly patients with vertebral fracture after vertebral body fusion.Methods A total of 74 middle-aged and elderly patients who underwent vertebral fusion surgery under general anesthesia and were equipped with a patient-controlled intravenous analgesia pump were selected from the Second Affiliated Hospital of Fujian Medical University from August 2021 to February 2023 were collected,and were randomly divided into two groups using a random number table method.In the control group,patients with stomach discomfort such as nausea and vomiting were given psychological counseling,distracted attention,strengthened patrol observation,and adjusted the use of analgesic pump or used drugs to stop vomiting according to the doctor′s advice when necessary.On the basis of the control group,the observation group was treated with bilateral valley-closing and acupuncture at Neiguan immediately and 12 hours after operation.Then observe and compare the severity and incidence of nausea and vomiting in the two groups at 4 hours,8 hours,12 hours and 24 hours after operation.Results Within 24 hours after operation,the effective rate of patients in the observation group(81.08%)was significantly higher than that in the control group(54.05%),and the difference was statistically significant(P<0.05).The first vomiting time and vomiting duration in the observation group were significantly shorter than those in the control group(P<0.05).Conclusion Acupoint embedding therapy for middle-aged and elderly patients undergoing vertebral body fusion surgery under general anesthesia can effectively reduce the incidence of postoperative nausea and vomiting,and reduce the degree of nausea and vomiting,with less adverse reactions,which is worthy of further research and application.
