山西省某医院银屑病患者住院费用的影响因素分析

Analysis of Influencing Factors of Hospitalization Costs of Psoriasis Patients in a Hospital in Shanxi Province

目的探讨银屑病患者住院费用的分布情况并分析其影响因素,为制定控制银屑病住院费用、减轻患者经济负担的政策提供参考依据。方法回顾性分析2015年1月—2021年12月山西省某医院收治的4617例银屑病患者的病历资料,对基本信息及费用情况进行描述性分析。在单因素分析基础上,采用多元回归分析银屑病患者住院费用的影响因素。结果银屑病住院患者平均费用(11595.24±1457.37)元;银屑病住院患者的住院费用与药品费之间呈正相关(P<0.001),护理费用增加时住院费用也有所增加(P<0.001);单因素分析显示,不同性别、年龄、婚姻状况、医疗付费方式、住院天数、住院年份、银屑病分类与住院费用比较,差异有统计学意义(P<0.001);多因素分析显示,患者的住院天数、住院年份、年龄、性别、医疗付费方式和疾病分类是银屑病住院费用的主要影响因素(P<0.05)。结论医院应优化患者的住院费用结构,以减少无效住院天数。医生需要综合考虑患者的病情、体征以及家庭经济状况,选择适当的治疗方案,从而提高患者的生活质量。同时,银屑病患者应增强自我管理能力,积极缓解症状,以降低经济负担。

Objective To investigate the distribution of hospitalization expenses in patients with psoriasis and analyze its influencing factors,and to provide reference for formulating policies to control hospitalization expenses and reduce the economic burden of patients.Methods The medical records of 4617 patients with psoriasis admitted to a hospital in Shanxi Province from January 2015 to December 2021 were retrospectively analyzed,and the basic information and cost were descriptively analyzed.Based on univariate analysis,multiple regression was used to analyze the influencing factors of hospitalization expenses of psoriasis patients.Results The average cost of hospitalized patients with psoriasis was(11595.24±1457.37)yuan.The hospitalization cost of psoriasis patients was positively correlated with the drug cost(P<0.001),and the hospitalization cost also increased when the nursing cost increased(P<0.001).Univariate analysis showed that different gender,different age,marital status,medical payment methods,hospitalization days,hospitalization years,psoriasis classification and hospitalization costs were significantly different(P<0.001).Multi-factor analysis showed that the length of hospitalization,year of hospitalization,age,gender,medical payment method and disease classification were the main influencing factors of hospitalization cost of psoriasis(P<0.05).Conclusion The hospital should optimize the structure of hospitalization cost in order to reduce the invalid hospitalization days.Doctors need to consider the patient's condition,signs and family economic status,choose the appropriate treatment plan,so as to improve the quality of life of patients.At the same time,patients with psoriasis should enhance their self-management ability and actively alleviate symptoms to reduce the economic burden.
