摘要
偏头痛发病率高,严重影响患者生活质量。临床中应根据患者临床特征进行可能病因诊断,评估其严重程度和失能程度,为偏头痛患者制订精准个体化的治疗策略和慢病管理方案提供充分的依据。本文梳理了偏头痛诊断流程及其亚型、诱因等对药物治疗选择的作用;综述了新型偏头痛精准治疗药物和神经调控的循证医学证据;提出疼痛科医生有责任承担起各类慢性难治性偏头痛的诊治工作,熟练应用各类药物治疗,掌握各类微创介入治疗方案,尤其是神经阻滞、神经射频和神经电刺激植入,为患者提供更多选择,缓解其疼痛,从而改善其生活质量。
Migraine is highly prevalent and significantly impairs patients'life quality.In clinical practice,the diagnosis and treatment involves identifying potential causes based on patients'symptoms,assessing the severity and disability and providing a solid basis for precise individualized treatment strategies and chronic management plans.This article covers the diagnostic approach,migraine types and triggers influence on medication selection,and reviews evidence-based medicine for new precision treatments and neuromodulation.Painologists have a responsibility in diagnosing and treating chronic refractory migraine,by employing a wide range of drug therapies and mastering minimally invasive techniques such as nerve block,radiofrequency neuroablation,and neurostimulation implant to offer patients more options for pain alleviation and life quality improvement.
