头颈联合电子计算机断层扫描血管成像与颅脑核磁共振血管成像对脑动脉狭窄的诊断价值研究

摘要 目的探析头颈联合电子计算机断层扫描血管成像(computed tomographyangiography,CTA)、颅脑磁共振血管成像(magnetic resonance angiography,MRA)诊断脑动脉狭窄的应用价值。方法选取2020年6月—2023年6月济宁市第三人民医院接诊的疑似脑动脉狭窄的患者100例作为研究对象。所有患者均接受头颈联合CTA和颅脑MRA,并以数字减影血管造影(digitalsubtractionangiography,DSA)为参照,对不同方法的诊断价值进行比较。结果头颈联合CTA灵敏度、特异度、阴性预测值均高于颅脑MRA,差异有统计意义(P<0.05);而头颈联合CTA、颅脑MRA的阳性预测值、狭窄程度符合率差异无统计意义(P>0.05)。结论头颈联合CTA与颅脑MRA均可对脑动脉狭窄进行有效诊断,并对脑动脉狭窄程度进行进一步诊断,但头颈联合CTA相比颅脑MRA具有更高的灵敏度与特异度,可将其作为首选。 Objective To analyze the diagnostic value of head and neck computed tomography angiography(CTA)and cranial magnetic resonance angiography(MRA)in cerebral artery stenosis.Methods From June 2020 to June 2023,a total of 100 suspected cases of cerebral artery stenosis were selected from the third People's Hospital of Jining.They all underwent head and neck CTA and cranial MRA.Digital subtraction angiography(DSA)as a reference,the diagnostic value of different methods was compared.Results The sensitivity,specificity and negative predictive value of CTA were significantly higher than those of craniocranial MRA(P<0.05).There was no significant difference in the positive predictive value and compliance rate of stenosis between head and neck CTA and cranial MRA(P>0.05).Conclusion Both head and neck CTA and craniocerebral MRA can effectively diagnose cerebral artery stenosis and the degree of cerebral artery stenosis.However,head-neck CTA has higher sensitivity and specificity than craniocerebral MRA,so it can be used as the first choice.
