贫困地区基层医院开展超声乳化加人工晶体植入术治疗白内障的效果分析

Effect of Phacoemulsification and Intraocular Lens Implantation in the Treatment of Cataract in Basic Hospitals in Poor Areas

目的:探讨白内障患者采用超声乳化加人工晶体植入术治疗的临床效果。方法:回顾性分析2012年8月—2022年8月林西县医院收治的600例白内障患者,根据不同治疗方法分为摘除组(307例)和乳化组(293例)。摘除组采用小切口白内障囊外摘除加硬性人工晶体植入手术治疗,乳化组采用超声乳化加人工晶体植入术治疗。比较两组患者围手术期指标、前房深度、眼压、视觉质量以及并发症。结果:乳化组切口大小、术中出血量、手术时间低于摘除组,差异有统计学意义(P<0.05)。术后,两组患者前房深度与术前比较有小幅度上升,眼压与术前比较有大幅度下降(P<0.05);且乳化组前房深度比摘除组高,眼压比摘除组低,差异有统计学意义(P<0.05)。术后,两组患者看电视、夜间视力状况评分比术前大幅度提升(P<0.05),且乳化组看电视、夜间视力状况评分比摘除组高,差异有统计学意义(P<0.05)。乳化组并发症发生率比摘除组低,差异有统计学意义(P<0.001)。结论:白内障患者采用超声乳化加人工晶体植入术治疗的效果显著,术后并发症较少。

Objective:To investigate the clinical effect of phacoemulsification and intraocular lens implantation in cataract patients.Methods:Six hundred cataract patients admitted to Linxi County Hospital from August 2012 to August 2022 were retrospectively analyzed and divided into extraction group(307 cases)and emulsification group(293 cases)according to different treatment methods.The extraction group was treated with small incision extracapsular cataract extraction and hard intraocular lens implantation,and the emulsification group was treated with phacoemulsification and intraocular lens implantation.Perioperative indexes,anterior chamber depth,intraocular pressure,visual quality and complications were compared between the two groups.Results:Incision size,intraoperative blood loss and operative time in emulsification group were lower than those in extraction group,and the differences were statistically significant(P<0.05);after operation,the anterior chamber depth was slightly increased and the IOP was significantly decreased in both groups(P<0.05);the anterior chamber depth in emulsification group was higher than that in extraction group,and IOP was lower than that in extraction group,the difference was statistically significant(P<0.05);after operation,the scores of TV viewing and night vision in both groups were significantly improved compared with those before operation(P<0.05);the scores of TV viewing and night vision in emulsification group were higher than those in extraction group,and the difference was statistically significant(P<0.05);the incidence of complications in the emulsification group was lower than that in the extraction group,and the difference was statistically significant(P<0.001).Conclusion:The effect of phacoemulsification and intraocular lens implantation in cataract patients is remarkable,and the postoperative complications are less.
