喉罩与气管插管全身麻醉在腹腔镜胆囊切除手术中的应用效果对比

Comparison of Effect of Laryngeal Mask and Tracheal Intubation General Anesthesia in Laparoscopic Cholecystectomy

目的:探讨喉罩与气管插管全身麻醉两种麻醉方式在腹腔镜胆囊切除手术中的应用价值。方法:选取2022年8月—2023年8月于马鞍山市和县人民医院麻醉科拟行腹腔镜胆囊切除手术患者70例作为研究对象,随机分为喉罩组(实施喉罩全身麻醉)与插管组(实施气管插管全身麻醉),各35例。比较两组麻醉效果。结果:麻醉后,两组收缩压(SBP)、舒张压(DBP)、心率(HR)水平均升高,喉罩组SBP、DBP、HR水平低于插管组,差异有统计学意义(P<0.05);喉罩组麻醉起效时间、阻滞完全时间、恢复自主呼吸时间、睁眼时间、定向力恢复时间均短于插管组,差异有统计学意义(P<0.001);喉罩组不良反应发生率低于插管组,差异有统计学意义(P=0.024)。结论:与气管插管全身麻醉相比,采取喉罩全身麻醉方式有助于维持腹腔镜胆囊切除手术患者血流动力学稳定,麻醉起效及术后恢复更快,安全性较高。

Objective:To investigate the application value of laryngeal mask and tracheal intubation general anesthesia in laparoscopic cholecystectomy.Methods:A total of seventy patients who were to undergo laparoscopic cholecystectomy in the Department of Anesthesiology,Maanshan Hexian County People's Hospital from August 2022 to August 2023 were selected as the study objects,and were randomly divided into laryngeal mask group(laryngeal mask general anesthesia)and intubation group(tracheal intubation general anesthesia),with thirty-five cases in each group.The anesthetic effect of the two groups was compared.Results:After anesthesia,the levels of systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)were increased in both groups,and the levels of SBP,DBP and HR in laryngeal mask group were lower than those in intubation group,with statistical significance(P<0.05);the effective time of anesthesia,the complete time of block,the time of spontaneous breathing recovery,the time of eye opening and the time of orientation recovery in laryngeal mask group were shorter than those in intubation group,and the differences were statistically significant(P<0.001);the incidence of adverse reactions in laryngeal mask group was lower than that in intubation group,the difference was statistically significant(P=0.024).Conclusion:Compared with tracheal intubation general anesthesia,laryngeal mask general anesthesia is helpful to maintain hemodynamic stability of patients undergoing laparoscopic cholecystectomy,and the anesthetic effect and postoperative recovery are faster,with higher safety.
