摘要
目的:分析鼻塞式持续气道正压通气联合氨溴索治疗新生儿肺炎的临床效果。方法:选取2020年9月—2022年9月遵义医科大学第二附属医院收治的新生儿肺炎患儿80例为研究对象,按随机数字表法分成对照组(鼻塞式持续气道正压通气治疗)、观察组(鼻塞式持续气道正压通气联合氨溴索治疗),各40例。比较两组患儿症状消退时间及住院时间、血气指标、不良反应。结果:观察组退热、止咳、肺部阴影消失时间及住院时间短于对照组,差异有统计学意义(P<0.001)。治疗前,两组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧合指数比较,差异无统计学意义(P>0.05);治疗后,两组PaO_(2)、氧合指数均上升,PaCO_(2)下降,观察组优于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:鼻塞式持续气道正压通气联合氨溴索治疗新生儿肺炎的效果较好,可改善患儿血气指标,促进症状消退,且未增加不良反应发生风险。
Objective:To analyze the clinical effect of nasal continuous positive airway pressure ventilation combined with ambroxol in the treatment of neonatal pneumonia.Methods:A total of eighty children with neonatal pneumonia admitted to the Second Affiliated Hospital of Zunyi Medical University from September 2020 to September 2022 were selected as the study objects,and were divided into control group(nasal continuous positive airway pressure treatment)and observation group(nasal continuous positive airway pressure combined with ambroxol)according to random number table method,with forty cases in each group.Symptom resolution time,hospital stay,blood gas index and adverse reactions were compared between the two groups.Results:The time of fever,cough,lung shadow disappearance and hospital stay in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.001).Before treatment,there was no significant difference in arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))and oxygenation index between the two groups(P>0.05);after treatment,PaO_(2)and oxygenation index increased and PaCO_(2)decreased in both groups,the observation group was better than the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Nasal continuous positive airway pressure combined with ambroxol is effective in the treatment of neonatal pneumonia,which can improve blood gas indexes and promote the regression of symptoms without increasing the risk of adverse reactions.
