摘要
目的:探析电子支气管镜肺泡灌洗治疗儿童重症肺炎合并低氧血症的效果。方法:选取2022年3月—2023年6月广西壮族自治区妇幼保健院收治的重症肺炎合并低氧血症患儿64例为研究对象,随机分为对照组和观察组,各32例。对照组采用常规治疗,观察组在对照组基础上联合电子支气管镜肺泡灌洗治疗。比较两组肺实变出现率、住院时间、症状缓解时间、血清炎性因子水平、治疗效果、并发症发生率。结果:观察组体温恢复正常用时、肺部啰音消失时间、咳嗽消失时间、住院时间均短于对照组,肺实变出现率低于对照组,差异有统计学意义(P<0.05);治疗后,两组超敏C反应蛋白、降钙素原水平均低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05);观察组总有效率高于对照组,差异有统计学意义(P=0.001);观察组并发症发生率低于对照组,差异有统计学意义(P=0.014)。结论:电子支气管镜肺泡灌洗治疗儿童重症肺炎合并低氧血症的效果显著,有助于促进患儿肺炎症状缓解,降低炎性指标,缩短住院时间,降低肺实变出现率,安全性较高。
Objective:To explore the effect of electronic bronchoscopic alveolar lavage on severe pneumonia complicated with hypoxemia in children.Methods:A total of sixty-four children with severe pneumonia complicated with hypoxemia admitted to Guangxi Zhuang Autonomous Region Maternal and Child Health Care Hospital from March 2022 to June 2023 were selected as the study objects,and were randomly divided into control group and observation group,with thirty-two cases in each group.The control group was treated with conventional treatment,and the observation group was treated with electronic bronchoscopic alveolar lavage.The incidence rate of lung consolidation,hospital stay,symptom remission time,serum inflammatory factor level,treatment effect and complication rate were compared between the two groups.Results:The time when body temperature returned to normal,the time when lung rales disappeared,the time when cough disappeared and the length of hospital stay in the observation group were shorter than those in the control group,and the incidence of lung consolidation was lower than that in the control group,the difference was statistically significant(P<0.05);after treatment,hypersensitive C-reactive protein and raw calcitonin in two groups were lower than before treatment,and observation group was lower than control group,the difference was statistically significant(P<0.05);the total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.001);the complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.014).Conclusion:Electronic bronchoscopic alveolar lavage has a significant effect on the treatment of severe pneumonia complicated with hypoxemia in children,which can help to alleviate the symptoms of pneumonia in children,reduce inflammatory indicators,shorten the length of hospital stay,and reduce the incidence of lung consolidation,with high safety.
