10例炎性线性疣状表皮痣临床及组织病理分析

Clinical and histopathological analyses of 10 cases of inflammatory linear verrucous epidermal nevus

目的:总结10例炎性线性疣状表皮痣(ILVEN)临床特征及组织病理特点。方法:采用回顾性分析10例ILVEN患儿的临床和组织病理资料。结果:10例患儿中,男2例,女8例,男女比例为1∶4;9例均表现为不同程度瘙痒,1例无自觉症状;7例皮损为局限性,3例为全身泛发,均表现为沿Blaschko线分布的疣状丘疹、斑块或鳞屑,局部皮损呈苔藓样改变。组织病理均表现为交替出现的角化过度及角化不全,角化过度区下方颗粒层增厚,角化不全区下方颗粒层减少或消失,3例出现典型Munro微脓肿,1例出现少量嗜酸性粒细胞。结论:该病临床少见,容易被误诊,结合临床及组织病理可早期诊断。

Objective:To summarize the clinical characteristics and histopathological features of 10 cases of Inflammatory Linear Verrucous Epidermal Nevus(ILVEN).Methods:The study employed a retrospective analysis of the clinical and histopathological data from 10 pediatric patients with ILVEN.Results:Among the 10 cases,there were 2 males and 8 females,with a male-to-female ratio of 1:4.Nine of the patients experienced varying degrees of itching,while 1 case reported no noticeable symptoms.Seven patients had localized skin lesions,whereas 3 had lesions that spread across the body.All cases presented verrucous papules or plaques distributed along Blaschko’s lines,with the affected areas exhibiting lichenification.Histopathologically,all cases showed alternating areas of hyperkeratosis and hypokeratosis;beneath the hyperkeratotic regions,there was thickening of the granular layer,whereas beneath the hypokeratotic regions,the granular layer was reduced or absent.Typical Munro microabscesses were observed in 3 cases,and 1 case showed a distribution of few eosinophils.Conclusion:This disease is uncommon in clinical practice and is prone to misdiagnosis;however,an early diagnosis can be made by combining clinical manifestations with histopathological examination.
