品管圈降低住院药房药品破损率的应用实践

Application practice of quality control circle in reducing drug damage rate in inpatient pharmacy

目的探索运用品管圈(QCC)的方法降低药房药品破损率。方法在药房内部选拔合适的人员组建品管圈,利用甘特图拟定整个品管圈活动的计划,利用帕累托图和二八法则确定降低药房药品破损率的改善重点,通过全员头脑风暴,运用鱼骨图的方式解析要因,制定对策并实施。运用PDCA循环法推进改善现状,比较改善前后药品破损率的降低值,以验证品管圈方法的有效性。对比QCC活动实施前后圈员的成长情况。结果通过本次品管圈活动,住院药房药品破损率有明显的下降,由原来的4.99‱降至2.12‱。目标达标率为95.34%,进步率为57.51%。结论品管圈活动能有效降低药房药品破损率,既减少了医院经济上的损失,节约资源,同时能够增强药师的责任心和成本意识。

Objective To explore the application of Quality Control Circles(QCC)in reducing drug breakage rates in pharmacy.Methods Within the pharmacy,a suitable team of individuals was selected to form a Quality Control Circle.A Gantt chart was employed to outline the entire plan for QCC activities.The Pareto and 80/20 rule were utilized to determine the key points of improvement for reducing drug breakage rates in pharmacy.Through brainstorming sessions involving all staff members,essential causes were derived by fish-bone diagrams,countermeasures were formulated,and then implemented.The PDCA(Plan-Do-Check-Act)cycle was employed to drive continuous improvement.The reduction value of drug breakage rates before and after the improvement was compared to validate the effectiveness of the QCC method.The growth of circle members was also assessed before and after the implementation of QCC activities.Results Through this QCC activity,there was a significant decrease in drug breakage rates in our hospital’s inpatient pharmacy,dropping from 4.99‱ to 2.12‱.The target achievement rate reached 95.34%,and the progress rate was 57.51%.Conclusion Quality Control Circle activities have proven effective in reducing drug breakage rates in pharmacy.Which not only reduces economic losses for the hospital and conserves resources,but also enhances pharmacists’sense of responsibility and cost-consciousness.
