星状神经节阻滞联合普瑞巴林治疗头面部急性带状疱疹的疗效及对神经痛的影响研究

Effect of stellate ganglion block combined with pregabalin on acute herpes zoster of head and face and its effect on neuralgia

目的研究星状神经节阻滞(stellate ganglion block,SGB)联合普瑞巴林治疗头面部急性带状疱疹(herpes zoster,HZ)的临床疗效及对带状疱疹后神经痛(post-herpetic neuralgia,PHN)的影响。方法将安阳市人民医院2022年1月至2023年12月76例头面部急性HZ患者按随机数字表法分为对照组和研究组,每组38例,对照组接受普瑞巴林治疗,研究组在对照组基础上接受SGB治疗,比较疗效。结果研究组止疱、结痂及愈合时间分别为(3.46±1.14)d、(9.28±1.84)d、(13.25±2.01)d,均短于对照组(P<0.05);治疗后1、4、8周,研究组疼痛程度低于对照组(P<0.05);治疗后1、4、8周,研究组睡眠质量优于对照组(P<0.05);治疗后4、12、24周,研究组PHN发生率低于对照组(P<0.05);治疗期间,研究组不良反应发生率(7.89%)低于对照组(28.95%)(P<0.05)。结论SGB联合普瑞巴林可提高急性HZ治疗效果,缓解疼痛,改善睡眠质量,降低PHN发生率,且安全性高,值得推广。

Objective To investigate the clinical efficacy of stellate ganglion block(SGB)combined with pregabalin for the treatment of herpes zoster(HZ)and herpes of post-herpetic neuralgia(PHN).Methods From January 2022 to December 2023,76 patients with acute HZ in the head and face of People's Hospital of Anyang City were divided into control group and study group according to random number table method,with 38 cases in each group.The control group received pregabalin treatment,while the study group received SGB treatment on the basis of the control group,and the efficacy was compared.Results The time of vesiculation,scab and healing in the study group were(3.46±1.14)d,(9.28±1.84)d and(13.25±2.01)d,respectively,which were shorter than those in the control group(P<0.05).At 1,4 and 8 weeks after treatment,the pain degree of the study group was lower than that of the control group(P<0.05).At 1,4 and 8 weeks after treatment,the sleep quality of the study group was better than that of the control group(P<0.05).The incidence of PHN in the study group was lower than that in the control group at 4,12 and 24 weeks after treatment(P<0.05).The incidence of adverse reactions in the study group(7.89%)was lower than that in the control group(28.95%)during treatment(P<0.05).Conclusion SGB combined with pregabalin can improve the treatment effect of acute HZ,relieve pain,improve sleep quality,and reduce the incidence of PHN,and it is safe and worthy of promotion.
