雷珠单抗治疗以扁平不规则色素上皮层脱离为特征的新生血管性慢性中心性浆液性脉络膜视网膜病变的效果

Efficacy of Ranibizumab in the Treatment of Neovascular Chronic Central Serous Chorioretinopathy Characterized by Detachment of the Flattened Irregular Pigment Epithelium

目的 观察玻璃体内注射雷珠单抗治疗以扁平不规则色素上皮层脱离(PED)为特征的新生血管性慢性中心性浆液性脉络膜视网膜病变(CSC)的临床效果。方法 回顾性分析2020年6月至2023年6月医院收治的100例(100眼)慢性CSC患者的病例资料,分为对照组和观察组,对照组接受口服安慰剂治疗,观察组接受玻璃体内注射雷珠单抗治疗。随访时间6个月。结果 观察组患者注射雷珠单抗(3.51±0.88)次。两组患者术前视力差异无统计学意义(P=0.892)。观察组术后6个月的视力优于对照组。观察组术后6个月的CRT与对照组差异有统计学意义(P<0.001)。观察组术后6个月的CRT较术前改善,差异有统计学意义(P<0.001)。两组患者治疗前后的扁平不规则PED最大高度、最大直径差异无统计学意义(P>0.999)。结论 雷珠单抗玻璃体内注射治疗以扁平不规则PED为特征的新生血管性慢性CSC疗效良好,但对扁平不规则PED无明显改善。

Objective To observation of the clinical efficacy of intravitreal injection of ranibizumab in the treatment of neovascular chronic central serous chorioretinopathy(CSC)characterized by flat irregular pigment epithelial detachment(PED).Methods Retrospective analysis of case data of 100 chronic CSC patients(100 eyes)admitted to the hospital from June 2020 to June 2023,divided into a control group and an observation group.The control group received oral placebo treatment,while the observation group received intravitreal injection of ranibizumab treatment.Follow up time was 6 months.Results The observation group received(3.51±0.88)injections of ranibizumab.There was no statistically significant difference in preoperative visual acuity between the two groups of patients(P=0.892).The visual acuity of the observation group was better than that of the control group at 6 months after surgery.The difference in CRT between the observation group and the control group at 6 months after surgery was statistically significant(P<0.001).The CRT of the observation group improved significantly 6 months after surgery compared to before,and the difference was statistically significant(P<0.001).There was no statistically significant difference in the maximum height and maximum diameter of flat irregular PED between the two groups of patients before and after treatment(P>0.999).Conclusion The intravitreal injection of ranibizumab has a good therapeutic effect on neovascular chronic CSC characterized by flat irregular PED,but there is no significant improvement in flat irregular PED.
