通元针法联合多西环素治疗反复种植失败合并慢性子宫内膜炎临床研究

Clinical Study on Tongyuan Acupuncture Therapy Combined with Doxycycline for Recurrent Implantation Failure Complicated with Chronic Endometritis

目的:观察通元针法联合多西环素治疗反复种植失败(RIF)合并慢性子宫内膜炎(CE)的临床疗效。方法:选取60例RIF合并CE患者,随机分为西药对照组与治疗组各30例。西药对照组连续口服盐酸多西环素片治疗1个月经周期。治疗组在西药对照组基础上接受通元针法治疗,1个月经周期为1个疗程,共治疗3个疗程。2组均在停药后2个月进行胚胎移植,并随访半年。随访期间记录并对比2组绒毛膜促性腺激素β(β-HCG)阳性率、临床妊娠率及胚胎种植成功率。比较2组上一次移植及本次移植时子宫内膜厚度的差值。结果:移植后第14天,β-HCG阳性率治疗组36.67%,高于西药对照组13.33%,差异有统计学意义(P<0.05)。移植后第28天,临床妊娠率、胚胎种植成功率治疗组分别为33.33%、20.41%,均高于西药对照组10.00%、6.00%,差异均有统计学意义(P<0.05)。治疗组2次移植时子宫内膜厚度差值大于西药对照组,差异有统计学意义(P<0.05)。结论:在口服多西环素的基础上加用通元针法治疗RIF合并CE,可有效促进子宫内膜生长,改善患者的妊娠结局。

Objective:To observe the clinical effect of Tongyuan Acupuncture Therapy combined with Doxycycline on recurrent implantation failure(RIF)complicated with chronic endometritis(CE).Methods:A total of 60 cases of patients with RIF complicated with CE were selected and randomly divided into the control group and the treatment group,with 30 cases in each group.The western medicine control group was given oral administration of Doxycycline Hyclate Tablets for 1 menstrual cycle,and the treatment group was additionally treated with Tongyuan Acupuncture Therapy based on the treatment of the western medicine control group.Both group were treated for 3 courses,1 menstrual cycle being 1 course of treatment.The two groups were given embryo transfer after 2 months of drug withdrawal and followed up for half year.During the follow-up,the chorionic gonadotropinβ(β-HCG)positive rate,clinical pregnancy rate and embryo implantation rate were recorded and compared between the two groups.Comparison of the difference in endometrial thickness between the two groups at the time of the last transplantation and at the time of the current transplantation.Results:On the 14 days after transplantation(IVF-ET),theβ-HCG positive rate was 36.67%in the treatment group,higher than that of 13.33%in the control group,the difference being significant(P<0.05).On the 28 days after transplantation,the clinical pregnancy rate and embryo implantation rate were respectively 33.33%and 20.41%in the treatment group,higher than those of 10.00%and 6.00%in the western medicine control group,differences being significant(P<0.05).The difference of endometrial thickness between last implantation and this one in the treatment group was greater than that in the western medicine control group,the difference being significant(P<0.05).Conclusion:Based on oral administration of Doxycycline Hyclate Tablets,the combination with Tongyuan Acupuncture Therapy for RIF complicated with CE can effectively promote the growth of endometrial membrane and improve the pregnancy outcome of patients.
