多层螺旋CT检查在周围型肺癌诊断中的应用价值分析

Analysis on Application Value of Multi-Slice Spiral CT in Diagnosis of Peripheral Lung Cancer

目的:探讨多层螺旋CT检查在周围型肺癌诊断中的应用价值。方法:选取2020年1月-2022年12月石门县中医医院收治的疑似周围型肺癌患者76例作为研究对象,均进行胸部X线检查、多层螺旋CT检查,以病理检查结果为“金标准”,比较两种检查方式诊断周围型肺癌的效能,不同病理类型周围型肺癌患者的多层螺旋CT征象。结果:病理检查结果显示,76例受试者确诊周围型肺癌57例,其中腺癌26例,鳞癌24例,腺鳞癌7例。多层螺旋CT检查诊断周围型肺癌的敏感度、特异度、准确率、阳性预测值、阴性预测值高于胸部X线检查,差异有统计学意义(P<0.05)。腺癌患者多层螺旋CT征象多为空泡征、磨玻璃征、深分叶征、空气支气管征、胸膜凹陷征,鳞癌多层螺旋CT征象多为毛刺征、支气管血管集束征。结论:多层螺旋CT检查诊断周围型肺癌的敏感度、特异度、准确率、阳性预测值、阴性预测值高于胸部X线检查,能清晰地显示病灶的特征,临床应用价值高。

Objective:To study the application value of multi-slice spiral CT in the diagnosis of peripheral lung cancer.Methods:A total of 76 patients with suspected peripheral lung cancer who were admitted to Shimen Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the study subjects.All patients underwent chest X-ray examination and multi-slice spiral CT examination.The results of pathological examination were used as golden standard to compare the diagnostic efficacy of the two examination methods for peripheral lung cancer.The multi-slice spiral CT signs of patients with different pathological types of peripheral lung cancer were compared.Results:The results of pathological examination showed that,among 76 subjects,57 cases of peripheral lung cancer were confirmed,including 26 cases of adenocarcinoma,24 cases of squamous carcinoma and 7 cases of adenosquamous carcinoma.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of multi-slice spiral CT in the diagnosis of peripheral lung cancer were higher than those of chest X-ray,and the difference was statistically significant(P<0.05).The multi-slice spiral CT signs of patients with adenocarcinoma were mostly vacuole sign,ground-glass sign,deep lobulation sign,air bronchial sign and pleural depression sign.The multi-slice spiral CT signs of squamous carcinoma were mostly spicule sign and bronchial vascular cluster sign.Conclusion:Multi-slice spiral CT has higher sensitivity,specificity,accuracy,positive predictive value and negative predictive value than chest X-ray,and can clearly display the lesions,and has high clinical application value.
